BlueCard Claim Filing: 5 Easy, Essential Steps
BlueCard claim filing trips up practices more often than almost any other Blue Cross task. The rule is simple: send the claim to the Blue plan in your own state, no matter which Blue plan issued the member’s card. This guide covers the five steps, the exceptions, and when a phone call is the only way to get an answer. It is written for psychotherapists, psychiatrists and the staff who bill for them.

What BlueCard Claim Filing Means

BlueCard is the program that lets Blue Cross and Blue Shield plans handle each other’s members. Blue Cross Blue Shield is not one insurer. The Blue Cross Blue Shield Association describes a system of independent, locally operated companies, and bcbs.com explains that BlueCard began in 1995 to link members to those local plans.
Under BlueCard, your local plan is your single point of contact. It takes the claim, sends it electronically to the member’s home plan, and the home plan processes it. Blue Cross and Blue Shield of Illinois and Blue Cross and Blue Shield of New Mexico both publish this rule in their provider guides.
Here is how it plays out in practice:
- A Georgia therapist sees a member whose card came from a Massachusetts Blue plan. The claim goes to the Georgia Blue plan. See our Anthem Georgia behavioral health billing guide.
- A Texas psychiatrist sees a member from Colorado. The claim goes to Blue Cross and Blue Shield of Texas. See the BCBSTX behavioral health billing guide.
- A Florida provider sees a member from Connecticut. The claim goes to Florida Blue. See Florida Blue claims and appeals.

BlueCard Claim Filing in 5 Steps
- Copy both sides of the ID card. Do it at the visit, not when the claim denies.
- Read the three-character alpha prefix. It sits at the start of the member ID and points to the home plan. Submit the prefix and ID exactly as shown. Do not add, drop or reorder characters. ID formats vary, and some run up to 17 positions.
- Verify eligibility and benefits. Use Availity or your local plan’s portal, or call the BlueCard eligibility line at 1-800-676-BLUE (2583). That number belongs to the Blue plans, not MCM South, and it covers eligibility, benefits and prior authorization only.
- Submit electronically to your local plan. Both the New Mexico and Illinois guides prefer electronic filing.
- Follow up with your local plan. It handles status checks, adjustments and inquiries, not the home plan.
When BlueCard Rules Do Not Apply
Private payer exceptions
These come from Blue plan provider guides. Plans differ, so confirm with yours.
- Federal Employee Program (FEP). Premera states that FEP claims are exempt from BlueCard and follow FEP billing guidelines. See FEP Blue and your local plan’s FEP guide.
- Overlapping service areas. Premera says providers in Idaho and Oregon are an exception because another Blue plan overlaps its area. Check the plan prefix list on the BlueCard resources page.
- More than one Blue contract. Blue Cross and Blue Shield of New Mexico says a provider who contracts with more than one Blue plan in a state for the same product type may file with either plan.
- Separate behavioral health vendor. Some plans and employer groups send mental health benefits to another company. If the card lists a separate behavioral health number, call it and ask where to send claims before you file.
Medicare, Medicaid and state rules
The steps above come from commercial BlueCard guidance. Medicare, Medicare Advantage and state Medicaid programs have their own filing rules. Check CMS.gov for Medicare and your state Medicaid site for Medicaid. Timely filing limits and prompt-pay rules vary by plan and by state, so take the deadline from your local plan’s provider manual.
Which Blue Plan Gets the Claim?
File with the Blue plan where you practice and hold your contract. The alpha prefix tells you which plan issued the card, but it does not change where you send the claim. If you are not yet contracted with your local Blue plan, start with credentialing and read in-network vs out-of-network before you bill.
When You Still Need to Call Blue Cross
Some answers only come by phone. Call your local plan for claims, and use the BlueCard line for eligibility. Have these ready:
- Your NPI and tax ID
- The full member ID with the alpha prefix
- Date of service, CPT code and billed amount
- The claim number, if one exists
Ask for a reference number and the representative’s name before you hang up. Plan phone lines change, so use our insurance companies contact directory and confirm the number on the plan’s own site. Looking for a longer take on the phone experience? Read our earlier post on calling Blue Cross. Unfamiliar terms are in the billing terms glossary.
BlueCard Claim Filing FAQ
What is the alpha prefix on a Blue Cross ID card?
It is the first three characters of the member ID. It identifies the member’s home Blue plan and routes the claim. Enter the prefix and ID exactly as printed on the card.
Do I send a BlueCard claim to the member’s home plan?
No. Send it to your local Blue plan. Your local plan routes the claim electronically to the home plan, which processes it, and your local plan stays your contact for status questions.
Can I use 1-800-676-BLUE to check claim status?
No. That line covers eligibility, benefits and prior authorization questions. Call your local Blue plan for claim status.
Do Federal Employee Program claims follow BlueCard rules?
Not always. One Blue plan’s provider manual says FEP claims are exempt from BlueCard and follow separate FEP billing guidelines. Check your local plan’s FEP guidelines before you file.
