EAP Billing for Therapists and Psychiatrists, Handled Correctly

EAP billing is how a provider gets paid for counseling sessions funded through an employee assistance program (EAP), an employer-paid benefit that runs separately from the client's health insurance.

What is an employee assistance program?

An employee assistance program is a workplace service that helps employers address productivity issues and helps employees identify and resolve personal concerns, through short-term counseling, assessment and referral.

The Employee Assistance Professionals Association (EAPA) lists short-term intervention and referral for diagnosis, treatment and follow-up among the core components of an EAP (EAPA, checked 10/1/26). The U.S. Office of Personnel Management describes the federal EAP as free, voluntary and confidential, with initial assessment and short-term counseling at no cost, often extended to immediate family, plus referrals for further services (OPM, checked 10/1/26). The U.S. Department of Labor describes EAPs as typically offering short-term mental health or substance abuse counseling or referral, along with financial and legal services (DOL release, checked 10/1/26).

The billing takeaway. An EAP is not health insurance. It is an employer-funded program run by a vendor, and the vendor, not the client's insurer, decides how you enroll, get authorized and get paid.

EAP billing is the process of filing claims for counseling sessions that an employee assistance program authorizes. It differs from billing a health plan because the EAP vendor sets the rules.

EAP billing differs from insurance billing in who pays, what the client owes, how sessions are authorized, and which form or portal the claim goes through.

What EAP vendors say in their own provider materials

These are examples from the vendors' provider pages, checked 10/1/26. Several of these documents show no date, so confirm the current version before you bill.

Treat those as Cigna's figures, not universal ones.
Carelon Behavioral HealthThe CAF-1 case activity and billing form is for EAP affiliates when services are preauthorized. The CAF-2 is for onsite EAPs and services that were not preauthorized. Submission is through ProviderConnect or by fax.
MagellanTo be paid for EAP services you must complete and submit the Employee Assistance Service Information (EASI) form.
ComPsychProviders apply with the Prospective Provider Interest Form. Once enrolled, they can log claims, check claim status, submit discharge information and request extensions of referral end dates.

Where EAP claims go wrong

  • Billed to insurance instead of the EAP. The claim can deny because the sessions were authorized under the EAP, not the client's insurance.
  • No authorization or EAP code. Vendors such as Cigna require the code before the first session.
  • Wrong form or portal. Carelon, Magellan and ComPsych each use their own process, so an 837P to the wrong place goes nowhere.
  • Sessions beyond the authorization. The employer's plan sets the cap. Billing past it means the sessions are unpaid, so track authorized visits.
  • Collecting a copay the program does not allow. Cigna's guide says EAP providers collect no out-of-pocket amounts. Check each vendor's rule.
  • Not enrolled with the vendor. Several vendors require you to apply to their network before you see their referrals.

Many EAP billing problems start with a missing authorization number or a session that goes past what the EAP approved. Check both before you submit. Clean electronic claims submission helps with the rest.

Psychotherapists and psychiatrists and EAP work

Psychotherapists

EAP programs center on assessment, short-term counseling and referral, so most EAP sessions are therapy sessions. Track the authorized session count and the vendor's form for each referral. See individual psychotherapy codes.

Psychiatrists

Because EAPs focus on short-term counseling and referral, medication management visits usually bill to the client's health insurance, not the EAP. Confirm what the vendor covers before you bill. See E/M codes and psychotherapy add-on codes.

Rules differ by EAP and by who sponsors it

Employer-sponsored EAPs (vendor contracts)

The vendor's provider agreement and manual control enrollment, authorization, forms and rates. They differ by vendor and by employer plan. Treat the vendor's own documents as the source of truth.

Federal employee EAPs

OPM describes the federal EAP as free, voluntary and confidential, with short-term counseling at no cost to the employee. For billing details, ask the EAP vendor your client's agency uses.

State rules

EAP billing is mostly set by vendor contract and employer plan, not by state Medicaid or state insurance rules. Where a state law affects a referral or a client's coverage after EAP sessions end, it varies by state. Do not apply one state's rule to another.

How do I get paid for EAP sessions as a therapist?

You usually have to enroll with the EAP vendor, receive an authorization or EAP code for each referral, and submit the claim the way that vendor requires. Each vendor sets its own forms and portals. Cigna, Carelon, Magellan and ComPsych all publish different provider instructions.

Do I bill an EAP on a CMS-1500 or through the vendor's portal?

It depends on the vendor. Carelon uses its CAF forms, Magellan uses the EASI form, ComPsych offers a provider portal, and Cigna's guide says to bill CPT code 99404. Because each vendor sets its own claim path, confirm it with the vendor before you bill.

What is an EAP authorization or case number?

It is the code or authorization the vendor issues for an employee's referral. In Cigna's guide, employer-referred clients must get an EAP code and give it to the provider before the first counseling session. Where the number goes on the claim depends on the vendor.

How many EAP sessions does a client get, and what happens after they run out?

The employer's plan sets the number, so it varies. Cigna's guide describes short-term programs of five to eight sessions, extendable up to twelve, but that is one vendor's structure. After the sessions end, the client may continue under their health insurance, which you should verify.

Do I need to be credentialed with an EAP vendor to see its clients?

Usually you need to enroll with the vendor. Carelon Behavioral Health takes applications through its Payer Space in Availity, and ComPsych takes a provider interest form. The enrollment path differs by vendor, so apply to each one directly.
