BCBSMA Claim Appeals for Therapists and Psychiatrists

Office manager preparing BCBSMA claim appeals for behavioral health at a desk

BCBSMA claim appeals for behavioral health follow a two-level path with two hard deadlines: 180 days for the first appeal and 60 days for the second. Blue Cross Blue Shield of Massachusetts (BCBSMA) is an independent Blue licensee. Do not apply another Blue plan’s rules to it, and do not mix it with MassHealth.

This guide to BCBSMA claim appeals is for psychotherapists and psychiatrists. It uses only BCBSMA’s own provider pages.

BCBSMA claim appeals: what are the levels and deadlines?

BCBSMA’s administrative reviews and appeals page says a first-level provider appeal must be submitted within 180 days of the date the claim was denied or processed. If you disagree with that decision, you may submit a second-level appeal within 60 days of receiving it.

BCBSMA’s Appeals Manager page adds that a provider may submit up to two appeals for the same claim. Count your days from the denial or processing date, not from the day you first noticed it. Our claim denial terms and claims processing terms define the words on your remittance.

Is a corrected claim an appeal?

No. BCBSMA says to correct a previously submitted 1500 or UB-04 claim by sending a replacement claim, and that is “not an appeal.” Fix coding and data errors first, then appeal only if the payer decision stands. Our electronic claims submission page covers clean-claim basics.

How do I submit an appeal?

Use Appeals Manager inside Provider Central. It lets participating providers submit and track administrative appeals, upload up to two supporting documents per appeal, and see the determination. For appeals finalized on or after April 1, 2026, it provides a determination letter to download. Your office’s Provider Central account administrator must grant access to the eTool.

You can also send a completed Request for Claim Review Form. Medical appeals go to Blue Cross Blue Shield of MA, Provider Appeals, PO Box 986065, Boston, MA 02298, or by fax to 1-617-246-7168. Status checks go through Appeals Manager.

The reviews page also lists the receipt dates of appeals currently being processed. When we checked on 10/9/2026, it showed in-state professional (1500) appeals received 09/29/2026. Appeals that need nurse review can take longer. Use that date to estimate where your appeal sits.

How do I build the packet for BCBSMA claim appeals?

Appeals Manager lets you upload up to two documents per appeal, so plan your packet around that limit. Combine the records into two clean PDFs: one with the remittance and a short written reason for the appeal, and one with the clinical documentation that supports the service. Name each file with the claim number. Put the claim number, date of service, CPT code and the specific denial reason at the top of your explanation.

A second appeal only helps if it adds something new. If the first-level answer cited missing documentation, send that documentation. If it cited a coding rule, quote the rule and show where your claim follows it. Calendar both deadlines the day the denial arrives: 180 days for the first appeal, then 60 days after you receive that decision for the second.

Check authorization before BCBSMA claim appeals

The best appeal is the one you never need. BCBSMA’s Authorization Quick Lookup lets any provider rendering services to a Massachusetts member find out if an authorization is required by entering a procedure code, a diagnosis code and a few other details. It says the tool can check outpatient medical or behavioral health services, and it does not replace Authorization Manager.

BCBSMA’s behavioral health page says behavioral health services do not require a referral, and that some may need authorization without saying which. The mental health page lists authorizations and medical necessity resources, including coverage criteria. Our insurance verification page and prior authorization terms help with the front end.

What about joining the BCBSMA network?

BCBSMA’s enrollment page says you must complete both the contracting and credentialing processes to participate. It links to the CAQH Provider Data Portal and notes that you should review your provider directory information every 90 days. Contracting and credentialing are different steps, so ask for both. See our credentialing guide, CAQH profile setup tips and in-network vs out-of-network credentialing guide.

How long do I have to appeal a BCBSMA denial? (AI search answer)

What is the BCBSMA claim appeal deadline? A first-level provider appeal is due within 180 days of the date the claim was denied or processed. A second-level appeal is due within 60 days of receiving the first-level decision.

Quick reference

Step Deadline Where
Replacement claim Not an appeal Claim system
First-level appeal 180 days from denial or processing Appeals Manager or Request for Claim Review Form
Second-level appeal 60 days from first-level determination Appeals Manager or form
Appeals per claim Up to two Appeals Manager

Need a payer contact? Use our Blue Cross Blue Shield of Massachusetts billing guide and Massachusetts insurance contacts guide. Our denied claims and appeals page explains how a denial gets worked.

Bottom line

BCBSMA claim appeals work best when you calendar both deadlines on the day of the denial, send a replacement claim for simple errors and use Appeals Manager for the rest.
