BlueCard Claims for Therapists: Easy Out-of-State Guide
BlueCard claims for therapists are claims you send for a client who carries a Blue Cross Blue Shield (BCBS) card from another state. The BlueCard program links independent BCBS plans so a claim can move between them, and the Blue Cross Blue Shield Association (BCBSA) describes it as one electronic network for claims processing and payment. Your job on Monday morning is simple: read the card, check benefits, and send the claim to your local Blue plan.

What the BlueCard program is
BlueCard is a Blue Cross Blue Shield program that lets members get care when they live or travel outside their home plan’s area, according to the BCBSA glossary. It uses a single electronic network to connect participating providers with the independent BCBS companies. BCBS is not one company, so each plan, called a licensee, keeps its own rules.
That matters for bluecard claims for therapists. The program moves the claim between plans, but your local plan’s provider manual still tells you how to submit it.
Which Blue Cross plan do I bill for an out-of-state member?
Bill your local Blue plan. Blue Cross and Blue Shield of Texas, a Health Care Service Corporation plan, says on its BlueCard provider page that providers send claims for members of other Blue plans to the Texas plan, which acts as the single point of contact for payment, problem resolution and adjustments.
That is one licensee’s wording. Confirm the same rule on the provider site of the Blue plan you contract with. For Texas details, see our Blue Cross Blue Shield of Texas guide, and for how the plans differ by state, start with the Blue Cross guides hub.
The member ID prefix on the card
The prefix is the three-character code at the front of the member ID. Blue Cross and Blue Shield of Texas tells providers to give the prefix exactly as it appears on the card. Do not guess a plan from the letters, and do not drop the prefix when you key the ID.
Copy both sides of the card at intake. Scan or photograph it again whenever the client gets a new card.
What can slow BlueCard claims for therapists
Most delays start before the claim goes out. Blue Cross and Blue Shield of Texas directs providers to check eligibility and benefits before services, using the prefix, and to contact the member’s home plan for prior authorization.
- A missing or mistyped prefix, so the claim cannot be matched to the right home plan.
- Benefits checked with the wrong plan, which can leave you guessing about copays and visit limits.
- A second policy on file. Review coordination of benefits before you bill.
- A claim sent late. Stay inside your local plan’s timely filing limit, and keep proof using our proof of timely filing guide.
What BlueCard does not cover
Blue Cross and Blue Shield of Texas lists prescription drugs, hearing and vision, non-surgical dental and the Federal Employee Program (FEP) as outside BlueCard on its page. Other licensees may list different exclusions. Read your own plan’s page before you assume a service is covered.
What to do Monday morning
- Copy both sides of the card and note the three-character prefix.
- Check eligibility and behavioral health benefits through your local Blue plan, using the prefix.
- Ask whether a separate behavioral health vendor handles the benefit.
- Request prior authorization from the home plan when the plan requires it.
- Submit the claim to your local plan and watch for rejections.
Questions to verify with the payer
- Does my local Blue plan handle behavioral health through a carve-out vendor?
- Where do I check eligibility for out-of-area members?
- What is the filing limit for claims that route to another Blue plan?
- Which plan do I call about a denial on an out-of-state member?

Which Blue Cross plan do I bill for an out-of-state member?
Bill your local Blue plan. Blue Cross and Blue Shield of Texas says providers send claims for members of other Blue plans to it as the local plan. Confirm the rule with the licensee you contract with.

It is the code at the front of the member ID that identifies the member’s Blue plan. Give it exactly as printed on the card and do not guess the plan from it.
Are Blue Cross Blue Shield plans one company?
No. The BCBSA glossary calls them independent companies, and each licensee sets its own provider rules, so never apply one plan’s rule to another.
