How Blue Cross Blue Shield Works: A Provider's Guide to the Blues

Blue Cross Blue Shield is not one insurance company. It is a federation of independent, locally operated companies that license the Blue Cross and Blue Shield names. Each company has its own payer ID, claims address, deadlines and provider manual. A rule that holds for Blue Cross of one state often fails in the next.

How the franchise model works

  1. The association owns the names. BCBSA manages the Blue Cross and Blue Shield trademarks. It grants licenses to independent companies to use them in exclusive geographic areas.
  2. Each company is its own insurer. It sets its own network, fee schedule, claim rules and credentialing. "Independent licensee" appears in its legal footer.
  3. Some companies share a parent. Elevance Health (the company formerly called Anthem, Inc.) holds the Blue license in 14 states: California, Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, New York, Ohio, Virginia and Wisconsin (Elevance 10-Q, March 2026). Sharing a parent does not make state rules identical.
  4. Brand names differ. You will see "Anthem Blue Cross and Blue Shield," "Florida Blue," "BlueCross BlueShield of Tennessee," "Excellus BlueCross BlueShield" and others. All are Blues.

BlueCard: how out-of-area claims move

BlueCard is the program that lets a member of one Blue plan use care in another Blue plan's area. Terms to know:

  • Home plan. The Blue company that issued the member's coverage.
  • Host plan. The Blue company in the area where you treat the patient.

The flow: you file the claim with your local Blue plan, the host. It sends the claim electronically to the member's home plan, which decides it. Payment comes back through your local plan. Your local Blue plan is your single point of contact for payment, adjustments and problems (BCBS of Oklahoma BlueCard manual; BCBSIL BlueCard page).

Check eligibility. Call the BlueCard Eligibility line at 800-676-BLUE (2583). Enter only the three-character prefix, and it routes you to the member's home plan.

The three-character prefix

The alpha prefix is the first three characters of most Blue member IDs. It tells the system which plan owns the member. The BlueCard manual says the prefix is critical for routing electronic transactions to the right Blue plan. Enter the full ID exactly as printed, prefix included. Dropping or changing it is a common cause of rejections.

Special cases

  • Federal Employee Program (FEP). BCBSA holds the federal contract for the Service Benefit Plan on behalf of participating Blue plans (OPM Office of Inspector General audit). Texas example: BCBSTX's FEP guide lists payer ID 84980 and a 365-day filing limit for that plan. Confirm yours.
  • National accounts and self-funded plans. National accounts, including self-funded employers, also process through BlueCard and are identified by prefix. Self-funded plans may follow the employer's rules, not the Blue's.
  • Behavioral health vendors. Some Blues hand behavioral health to a vendor. Carelon Behavioral Health manages it for Anthem in Connecticut and Colorado. BCBS of Texas, BCBS of Illinois and BCBS of Tennessee describe it as in-house. See each state guide.

Why this matters for your practice

  • Payer ID. Different company, different ID. Never assume.
  • Claims address and deadline. They vary by licensee. Examples we confirmed: Texas 365 days, Illinois 180 days, Tennessee six months.
  • Credentialing. Each company runs its own process.
  • Behavioral health rules. In-house at some Blues, vendor-run at others.
  • State law. The state insurance regulator for fully insured plans changes with the state.

Private payer, Medicare and Medicaid rules also differ. Blue Medicare Advantage and Blue-affiliated Medicaid plans have their own payer IDs, addresses and rules.

What to do Monday morning

  1. Read the three-character prefix on the card.
  2. Verify eligibility through Availity or 800-676-BLUE.
  3. File with your local Blue plan, even for out-of-state members.
  4. Find the licensee's state guide and read its payer ID, address and deadline.
  5. Do not copy a Blue rule from one state to another.
