TennCare Behavioral Health Billing: Register Once, Contract With Each Plan

Billing specialist reviewing a Medicaid claim on a computer screen for TennCare behavioral health billing

TennCare behavioral health billing starts with one state registration and then splits into separate contracts with each managed care organization (MCO). There is no single TennCare claims desk for therapy or psychiatry. Each plan pays its own claims under its own rules, so you need to know which plan your patient is in before you bill.

What is TennCare behavioral health billing, and who pays?

TennCare is Tennessee’s Medicaid program. Its Behavioral Health Services division oversees mental health and substance use disorder services for TennCare members, and the covered list includes psychotherapy and medication management when medically necessary (TennCare behavioral health services, page modified 2026-10-02).

Three managed care organizations deliver those benefits: Wellpoint, BlueCare Tennessee and UnitedHealthcare Community Plan. TennCare’s contractor page also lists TennCare Select (TennCare managed care contractors). For a quick state overview and manual links, see our Tennessee Medicaid billing page.

Step 1: How do I register as a TennCare provider?

Register with the state first. TennCare says a valid TennCare/Medicaid ID number is required to participate. The ID is also used to contract with a TennCare MCO, submit Medicare/Medicaid crossover claims and verify member eligibility (TennCare provider registration).

Registration does not put you in a plan’s network. That is the next step. Contracting plan by plan is the core of TennCare behavioral health billing.

Step 2: Do I contract with TennCare or with each MCO?

You contract with each MCO separately. TennCare’s contractor page says each MCO creates its own provider contracts, keeps its own fee schedules, processes its own claims and maintains its own network. For fee schedules or network questions, contact the provider relations team at that plan.

TennCare lists its plans by West, Middle and East region, so confirm which region you will serve when you apply. Our credentialing page explains the application habits that apply to every plan.

How do I know which plan to bill for a TennCare patient?

Bill the MCO the member is enrolled in on the date of service. Check eligibility before each visit rather than relying on the card from last month, because eligibility can change. TennCare’s registration page names eligibility verification as one use of your TennCare ID.

Build the habit into intake. Our insurance verification page lists what to capture, and the Tennessee insurance contact guide points to payer lines. Plan-specific rules are what make TennCare behavioral health billing different from commercial billing.

Medicaid rules: what to watch (TennCare only)

This section covers TennCare. Commercial plans, including BlueCross BlueShield of Tennessee’s commercial networks, are different.

Each plan sets its own manual. TennCare’s contractor page says each MCO keeps its own fee schedule and claims process. Read the manual for the plan on the member’s card, and never assume the rule you learned from one MCO carries to another. Our Medicaid billing terms glossary defines terms such as crossover claims and managed care.

Crossover claims have a clock. If your patient has both Medicare and TennCare, the Medicaid plan may pay a share after Medicare. BlueCare Tennessee’s provider newsletter says providers have 365 days from the date of service, or 180 days from the Medicare explanation of payment date, whichever is greater (BlueAlert, August 2026). Another plan may set a different window.

Corrected and rejected claims need a plan-specific path. The same BlueCare newsletter says rejected, returned or denied claims must be resubmitted within six months. See our electronic claims submission page for clean-claim habits and our denied claims and appeals page for follow-up.

What changes for psychiatrists and psychotherapists?

Psychotherapists mostly bill time-based codes, covered in our psychotherapy codes guide. Psychiatrists bill evaluation and management (E/M) visits and may add psychotherapy add-on codes. Check each MCO’s manual for how it handles those combinations, and ask provider relations if the manual is silent. Home and community codes carry different meanings by state, so use our Medicaid HCPCS guide before you bill any H-code. Prescribers and therapists alike should build these checks into TennCare behavioral health billing.

Common TennCare claim problems

  • Billing the wrong plan. The member’s plan on the date of service decides the payer. Check eligibility each visit.
  • Using one MCO’s manual for all three. TennCare’s own page says each MCO keeps its own fee schedule and claims process.
  • Missing a crossover window. Dual-eligible patients (Medicare plus TennCare) add a second payer and a second deadline.
  • Skipping the registration step. Without a TennCare ID, you cannot contract with an MCO or submit crossover claims, per TennCare.

A simple weekly habit helps: pull every unpaid TennCare claim older than two weeks and check which plan holds it. A short weekly review keeps TennCare behavioral health billing on track.

Frequently asked questions

Does TennCare have one claims address for behavioral health?

No. Each MCO processes its own claims, according to TennCare’s contractor page.

Do I need a TennCare ID before I contract with an MCO?

Yes. TennCare lists contracting with an MCO as a use of the TennCare/Medicaid ID, and requires the ID to participate.

Which plans cover TennCare behavioral health?

Wellpoint, BlueCare Tennessee and UnitedHealthcare Community Plan, per TennCare’s behavioral health page.

How long do I have to bill a crossover claim to BlueCare?

BlueCare Tennessee’s August 2026 newsletter says 365 days from the date of service or 180 days from the Medicare payment date, whichever is greater. Check the plan’s current manual.
