MassHealth Provider Enrollment for Therapists and Psychiatrists

Therapist completing MassHealth provider enrollment paperwork at a desk

MassHealth provider enrollment is how a Massachusetts therapist or psychiatrist becomes a participating Medicaid provider so claims can be paid. MassHealth is the state’s Medicaid program, run by the Executive Office of Health and Human Services (EOHHS). This guide covers the application steps, the two enrollment types, a new claim rule for 2027 and the separate credentialing that managed care plans require. Status is current as of October 15, 2026. Confirm each item on mass.gov before you act. This is operational guidance, not legal advice.

How MassHealth provider enrollment works

The apply to become a MassHealth provider page lays out the process. Review the MassHealth regulations and provider manuals first. Then request an application through the Provider Application Request Form. MassHealth emails instructions, and an application fee may apply. Complete the application and send it in. Mailed applications go to MassHealth Provider Enrollment, P.O. Box 278, Quincy, MA 02171.

You must be an approved participating provider before you submit claims. For help, call (800) 841-2900 (TTY 711).

Use the right portal for each task. Per the MassHealth provider portals page, the Provider Online Service Center (POSC) handles claims, claim status, prior authorizations and eligibility. Provider Self-Service handles enrollment and credentialing tasks, and you can check your application status there with your Application Tracking Number or NPI (National Provider Identifier).

Fee-for-service or ORP: pick the right MassHealth provider enrollment type

The MassHealth enrollment and credentialing FAQ, updated August 6, 2025, explains the two paths:

  • Fee-for-service (FFS): individuals who practice independently or in a group and bill MassHealth directly.
  • Ordering, referring and prescribing (ORP): other individuals who meet ORP criteria. The FAQ says ORP enrollment is a condition of holding a Massachusetts license.

Groups enroll each service location where they see members. Administrative offices cannot be enrolled. A pending license is held for up to six months, and signed forms cannot be accepted after 90 days. Tell MassHealth at least 14 days before a change.

The 2027 claim rule for MassHealth provider enrollment

All Provider Bulletin 421, dated October 1, 2026, sets a new rule. For dates of service on or after January 1, 2027, any provider required on a claim must be enrolled with MassHealth, or the claim is not payable. That includes ordering, referring, prescribing, attending, supervising and operating providers. Billing providers must make sure those providers are enrolled. The bulletin’s list covers psychologists, licensed independent clinical social workers and psychiatric clinical nurse specialists. Read the bulletin and check your whole team’s enrollment before year end.

Managed care plans credential you separately

MassHealth members are often in a managed care plan. MassHealth’s list of ACOs and MCOs names 15 accountable care organizations (ACOs), 2 primary care ACOs and 1 managed care organization (MCO), WellSense Essential. Tufts Health Together MCO was discontinued effective January 1, 2026, per All Provider Bulletin 410.

MassHealth provider enrollment does not put you in a plan’s network. Each plan runs its own credentialing. WellSense says credentialing through Carelon does not count: you must be credentialed with WellSense and grant it access in CAQH. See the WellSense behavioral health FAQ. Read our credentialing and network status guide to see how enrollment and network status differ.

WellSense behavioral health changes to watch

WellSense now administers behavioral health for its products. For Massachusetts and New Hampshire products other than New Hampshire Medicaid, dates of service on or after January 1, 2026 go to WellSense, and earlier dates go to Carelon. Inpatient claims are excluded. WellSense also says that beginning September 1, 2026, behavioral health claims submitted without the required taxonomy codes will be denied, and it does not accept faxed claims. See the WellSense behavioral health page. Check that your taxonomy code is on every claim.

MassHealth, Medicare and private insurers are different workflows

MassHealth fee-for-service claims have a 90-day initial filing limit, per the MassHealth billing timelines. Medicare allows one calendar year. Private insurers set filing limits in your contract. Keep the three in separate checklists. For private insurer numbers, use our Massachusetts insurance companies contact guide, and see how the 45-day rule works in our Massachusetts prompt pay law guide.

FAQs

How do I start MassHealth provider enrollment?

Request an application through the Provider Application Request Form on mass.gov, complete it and submit it. You must be an approved participating provider before you send claims.

What is the difference between FFS and ORP in MassHealth provider enrollment?

FFS providers bill MassHealth directly. ORP providers order, refer or prescribe but do not bill MassHealth directly.

What changes for MassHealth claims on January 1, 2027?

Under All Provider Bulletin 421, providers required on a claim, such as ordering, referring, prescribing or attending providers, must be enrolled with MassHealth or the claim is not payable.

Does MassHealth provider enrollment put me in the WellSense network?

No. WellSense runs its own credentialing, and Carelon credentialing does not count toward it.
