Private payer: appeals
Medical provider appeals go to Blue Cross Blue Shield of MA, Provider Appeals, PO Box 986065, Boston, MA 02298 (fax 1-617-246-7168). File a first-level appeal within 180 days of the claim decision, and a second-level appeal within 60 days of the first-level decision. Send corrections as replacement claims, not appeals (BCBSMA reviews and appeals).

Private payer: credentialing
Many behavioral health clinicians (child psychiatrists, clinical psychologists, LICSWs, LMFTs, LMHCs and LADCs) share one BCBSMA application. New solo practices and groups also complete the Behavioral Health Professional Practice Application (BCBSMA contracting applications).
Behavioral health authorization
