Anthem Midwest Mental Health Billing: 5 Easy Essentials
Anthem Midwest mental health billing means getting behavioral health claims paid by Anthem Blue Cross and Blue Shield in Indiana, Kentucky, Missouri, Ohio and Wisconsin. Elevance Health holds the Blue license in all five, according to its 2025 annual report. In these states a second company, Carelon Behavioral Health, manages much of the provider side, so the first job is knowing who does what.
Anthem Midwest mental health billing: who credentials and who pays
Anthem pays the claim and Carelon manages the behavioral health network. Carelon’s own quick reference guides for Anthem commercial and Medicare Advantage plans point providers to the same credentialing line, the Carelon National Provider Service Line (1-800-397-1630). Claims go through Availity or your clearinghouse. Missouri is a partial state: the annual report says Anthem’s Missouri license excludes 30 Kansas City-area counties, so check the member’s county.
| State | Carelon guide | Claims | Authorization |
|---|---|---|---|
| Kentucky | Anthem commercial and Medicare Advantage (February 2023) | Availity | Availity Interactive Care Reviewer, or the number on the member card |
| Missouri | Anthem commercial and Medicare Advantage (2.7.2023) | Availity or clearinghouse | Number on the member card; requirements on anthem.com |
| Ohio | Anthem commercial and Medicare Advantage (no date printed) | Availity; paper address on the guide | Availity Interactive Care Reviewer, or the card number |
| Wisconsin | Anthem commercial and Medicare Advantage (2.7.2023) | Availity or clearinghouse | Number on the member card |
These guides are old. Treat them as a starting map and confirm each detail with Anthem before you bill.

Who do I bill for Anthem mental health in the Midwest?
Bill the plan on the member’s card, using the routing your eligibility check returns. Run the check in Availity first and write down who manages mental health benefits. If a vendor is named, ask how to submit claims, authorizations and appeals through that vendor. Good Anthem Midwest mental health billing depends on that one answer.
Private payers
For Indiana, Anthem’s behavioral health page (August 28, 2025) says Carelon now manages Anthem’s in-network behavioral health providers, at the same reimbursement rates. Anthem still administers payment, and claims and clinical processes stay the same. The page also says the Carelon provider agreement date was extended and a revised agreement would follow, so watch your mail and email. For Anthem Midwest mental health billing, your contract sets your filing and appeal windows in every state. Read it and keep the numbers where billing staff can see them. Our in-network and out-of-network credentialing guide explains why network status changes the rules.
Medicare and Medicaid
Indiana Medicaid has published guidance for this arrangement. In its 2026 seminar slides (July 2026), the state’s Medicaid program (IHCP) says Anthem keeps member operations, claims support and payment, while Carelon manages the delegated behavioral health network, including credentialing and enrollment. New claims are due 90 calendar days from the date of service. Corrected claims and disputes are generally due 60 days from the explanation of benefits date, though the slides disagree on the start date, so confirm it. The professional payer ID shown is 00630. Medicare Advantage plans are covered in the commercial guides above.
State rules
Each of the five states sets its own prompt-pay and appeal rules, and they are not the same. Ask the state insurance department for the rule that applies to your plan type, and ask your counsel if a large payment is at stake.
Where Anthem Midwest mental health billing denials start
Most denials trace to routing, authorization or documentation. A claim goes to the wrong company. An authorization was never recorded. A note does not support the service billed. Read the remittance codes first (our guide to CARC and RARC codes shows how), then compare the denial to your contract. Psychiatrists should also confirm that evaluation and management visits and psychotherapy add-on codes go to the same administrator.
| Check | Ask before billing |
|---|---|
| Plan and county | Is the member inside Anthem’s service area (Missouri)? |
| Behavioral health administrator | Anthem or Carelon for this plan? |
| Credentialing | Is my record current with Carelon? |
| Authorization | Needed, and where is the number stored? |
| Deadlines | Filing and appeal windows in my contract? |
