State Insurance Contact Guides: 22 Essential Provider Lists

State insurance contact guides put the provider contacts for one state on one page: each major payer, the state Medicaid agency and your Medicare claims contractor. MCM South now has 22 of them, and they are written for psychotherapists, psychologists and psychiatrists who are tired of hunting for the right line.

State insurance contact guides: what each one includes

  • Provider contact lines for the main private payers, labeled by purpose and checked in October 2026
  • A link to each payer’s official provider page next to its row
  • The state Medicaid agency and its claims contractor
  • The Medicare Administrative Contractor that processes your Part B claims
  • A short list of what to have ready before you call
  • Free CPT quick sheets for common behavioral health codes

When we cannot confirm a number on a payer’s own page, the guide sends you to that payer’s contact page instead of printing a number we cannot support.

How to use state insurance contact guides before you call

Start with the right plan. Check the member ID card, then open your state’s guide and find the payer. Have your NPI and tax ID, the patient’s ID and date of birth, the date of service and the claim number in front of you. Our insurance verification service does this check up front for MCM South clients. For a claim that was denied, our guide to CARC and RARC codes explains what the payer told you.

The 22 state insurance contact guides

Guides are live for Alabama, Arizona, California, Colorado, Connecticut, Florida, Georgia, Illinois, Maryland and DC (one guide), Massachusetts, Michigan, Minnesota, New Jersey, New York, North Carolina, Ohio, Oregon, Pennsylvania, Tennessee, Texas, Washington and Wisconsin. Spanish versions are available for the guides. Start from the insurance companies contact by state hub, or go straight to Georgia, Massachusetts, Texas or Florida.
