Minnesota insurance companies contact: private payer provider numbers
Call the provider services number, not the member line. Each row below shows the number, what it is for and the insurer page it came from. If a payer has no behavioral health number listed, use the number on the member ID card.

Minnesota Medicaid provider enrollment
The Minnesota Department of Human Services runs Minnesota Health Care Programs (MHCP), which is the state Medicaid program. Providers enroll through the DHS MHCP enrollment page: MHCP provider enrollment. Many members are in managed care plans such as Blue Plus, HealthPartners, Medica and UCare, so confirm the member’s plan before you call. Checked October 2026.
Minnesota Medicare: who processes your Part B claims
Minnesota Medicare Part B claims go to one Medicare Administrative Contractor (MAC). National Government Services is the MAC for Jurisdiction 6, which covers Illinois, Minnesota and Wisconsin. Medicare Advantage plans are private plans, so call the number on the member card for those. Medicare, Medicaid and private payers follow different rules. Use the CMS MAC page for the current contractor contact details.

What to have ready before you call a Minnesota insurer
A short, prepared call is faster than a long one. Keep these in front of you before you dial any Minnesota insurance companies contact number:
- The patient’s name as it appears on the card, date of birth and address.
- Your individual NPI (National Provider Identifier) and your tax ID.
- The date of service, the CPT code and the total billed amount. See the non-E/M CPT guide and the E/M CPT guide for psychiatrists.
- The claim number from the payer portal, if you have one.
- Medicare Part B questions: have your NPI, PTAN and the last five digits of your tax ID ready.
- Minnesota Health Care Programs members: confirm which plan (Blue Plus, HealthPartners, Medica, UCare or fee-for-service) the member is in before calling, since each plan has its own provider line.
Write down the name of the representative and a call reference number. Phone menus, holds and transfers can add up, and a dropped call means starting over. If you would rather not make these calls, MCM South does it for psychotherapists and psychiatrists.
