Franklin Mental Health Billing Service: 5 Easy Checks

A Franklin mental health billing service is an outside company that sends insurance claims, posts payments and follows up on denials for therapy and psychiatry practices in Franklin, Tennessee. It helps most when a growing group practice sees families and teens on plans held by a parent, takes a mix of in-network, out-of-network and private-pay clients, and has no one free to chase claims. This guide gives five checks to run, plus the Tennessee rules behind them.

Franklin: why a growing group practice feels the payer mix

The U.S. Census Bureau’s QuickFacts page for Franklin lists a median household income of $119,528 for 2020 to 2024 in 2024 dollars. That figure says nothing about any one client’s coverage. A Franklin mental health billing service should read each client’s card, not the zip code.

The plan belongs to a parent, not the client

When a practice sees children and teens, the person who holds the policy is often a parent. The claim must show the policyholder and the client as two different people, each with the right name, date of birth and member ID.

A mismatch there is a common cause of rejections. A Franklin mental health billing service should run insurance verification on the policyholder and the dependent before the first visit, and again when a new plan year starts.

Growing groups and changing rosters

Each new clinician starts with no payer status. Keep one record per clinician and payer with the effective date, and do not schedule a payer’s clients until that date is confirmed in writing. Our guide to credentialing for therapists explains the process.

Out-of-network and private-pay clients

Some families choose to use out-of-network benefits or pay privately. Decide which route each client is on at intake, and put it in writing. If a clinician is out of network with a plan, ask whether the service bills out-of-network claims. MCM South bills out-of-network claims for clients.

Psychiatrists add evaluation and management (E/M) visits, often with a psychotherapy add-on code. Our guide to psychiatry billing services covers the details. If you offer video visits, read our telehealth billing guide so the place-of-service code and modifier match the payer’s rule.

Five checks a Franklin mental health billing service should run

Run these five checks for every new clinician and every new payer, and again when a payer changes its rules.

CheckWhat to confirm
1. Payer enrollmentEach plan’s credentialing status and effective date, in writing, with BCBST treated as its own payer
2. Policyholder and client matchPolicyholder name, client name, date of birth, member ID and relationship on the claim match the card on the date of service
3. Network routeEach client is marked in-network, out-of-network or private-pay, and the consent or payment terms are on file
4. Claim setupBilling and treating clinician names, NPIs and tax ID match each payer’s file; psychiatry add-on codes sit with the right E/M code
5. Filing clocksMedicare’s 12 months, TennCare plan windows and each commercial contract’s own window

Ask what happens to visits before an effective date. Hold or reschedule those clients until the date is confirmed in writing.

Tennessee rules behind a Franklin mental health billing service

Keep private, public and state rules apart. A claim follows the rules of its payer type.

Private payers

The footer of the BlueCross BlueShield of Tennessee provider site reads “BlueCross BlueShield of Tennessee, Inc., an Independent Licensee of the Blue Cross Blue Shield Association” (copyright 1998-2026). Treat it as its own payer. Do not apply another state’s Blue plan rules to it.

The same site lists Availity log-in, coverage and claims, authorizations and appeals, and manuals and policies among its provider tools. Your contract controls the rest, so read it with our payer contract compliance guide, and keep payer contacts in our insurance companies contact directory.

Medicare and Medicaid

The Centers for Medicare & Medicaid Services (CMS) lists Palmetto GBA, LLC as the A/B Medicare Administrative Contractor for Jurisdiction J, which covers Alabama, Georgia and Tennessee. The page shows the contract as “Fully Implemented,” with an award date of August 29, 2024.

Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies.

The state’s TennCare provider page describes TennCare as the state’s managed Medicaid and Children’s Health Insurance Program agency. It says managed care organizations (MCOs) cover medical, behavioral and long-term care services, that each MCO “creates their own contracts with providers” and that each “processes their own claims.” Providers who serve TennCare members must enroll with TennCare first, then contract with an MCO. Ask each MCO for its claim filing window in writing.

State rules

The Tennessee Department of Commerce and Insurance describes its Insurance Division as “tasked with protecting consumers and ensuring a viable insurance marketplace in the State.” Its complaint page links a provider complaint form. The page does not describe prompt-pay deadlines, so confirm which rule covers a late claim with the payer or your counsel before you file anything.

How do I choose a Franklin mental health billing service for a group practice with mixed coverage?

Choose a Franklin mental health billing service that can show, for each clinician, which payers are active and from what date, and that bills in-network, out-of-network, Medicare and TennCare claims by the rules of each.

Ask for your clean claim rate, a list of denial reasons and a sample monthly report. Ask how the service reads remittances, since each denial code points to a fix. Use our guide to CARC and RARC codes to check its answers. Get fee terms in writing, and confirm in writing that the service bills in Tennessee.

What to do Monday morning

  1. List every clinician with each payer’s enrollment status and effective date.
  2. Add a column to your client list for policyholder name and relationship to the client.
  3. Mark each client in-network, out-of-network or private-pay, and check the paperwork is on file.
  4. Write each payer’s filing window next to its name, using your contract.
  5. Pull your last remittance and note the top three denial reasons.
  6. Send the five checks to each Franklin mental health billing service you are considering.

The short version: Franklin mental health billing service

A Franklin mental health billing service earns the job by confirming enrollment before claims go out, matching policyholder and client on every claim, treating BCBST as its own payer, routing Medicare and TennCare claims correctly and tracking every payer’s clock. Get the answers, reports and fees in writing.

FAQ

Does a billing service need to be in Franklin?

No. Claims and reports move electronically, so a service can work with practices in Franklin and across Tennessee. Tennessee payer knowledge, written terms and clear reporting matter more than an address. Ask the service to confirm in writing that it bills in Tennessee.

Is BlueCross BlueShield of Tennessee the same as other Blue plans?

No. Its provider site calls it an Independent Licensee of the Blue Cross Blue Shield Association. Use its own manuals and your contract, not another state’s Blue plan rules.

Whose information goes on a claim when a parent holds the plan?

The claim shows the policyholder and the client separately, with the client’s relationship to the policyholder. Verify both before the first visit and confirm the member ID matches the card.

How long do I have to file Medicare claims?

Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies. Claims go to the Medicare contractor for your state, which CMS lists as Palmetto GBA, LLC for Tennessee.

Who processes TennCare claims?

TennCare’s provider page says each managed care organization processes its own claims and creates its own provider contracts. Enroll with TennCare, contract with each MCO and get its filing window in writing.
