Knoxville Mental Health Billing Service: 5 Easy Checks
A Knoxville mental health billing service is an outside company that sends insurance claims, posts payments and follows up on denials for therapy and psychiatry practices in Knoxville, Tennessee. It helps most when a solo or small group practice in East Tennessee carries a mix of BlueCross BlueShield of Tennessee, other commercial plans, Medicare and TennCare and has no one free to chase claims. This guide gives five checks to run, plus the Tennessee rules behind them.
Knoxville: why East Tennessee practices need a clear payer map
The U.S. Census Bureau’s QuickFacts page for Knoxville lists a median household income of $54,039 for 2020 to 2024 in 2024 dollars. That figure says nothing about any one client’s coverage. A Knoxville mental health billing service should read each client’s card, not the zip code.
Solo and small group practices in East Tennessee
In a small practice, the owner often sees clients, answers the phone and handles billing. That works until a BlueCross plan, two commercial plans, Medicare and a TennCare plan each send different rules in the same month.
A Knoxville mental health billing service should keep each clinician’s status with each payer in one place. It should also run insurance verification before each first visit.
Clients near a university
If your caseload includes students, graduate students or university staff, do not guess the coverage. A client may be on a parent’s plan, an employer plan or a plan sold through the school. Each one has its own payer, member ID and claim address.
Ask for the card at intake and record the plan name, subscriber and effective date. Re-check at the start of each term, since coverage can change when a school year does.
Psychiatry and out-of-network visits
Psychiatrists bill evaluation and management (E/M) visits, often with a psychotherapy add-on code on the same visit. Check both lines together. Our guide to psychiatry billing services covers the details.
If a clinician is out of network with a plan, ask whether the service bills out-of-network claims. MCM South bills out-of-network claims for clients. If you offer video visits, read our telehealth billing guide so the place-of-service code and modifier match the payer’s rule.

Five checks a Knoxville mental health billing service should run
Run these five checks for every new clinician and every new payer, and again when a payer changes its rules.
| Check | What to confirm |
|---|---|
| 1. Commercial and Blue enrollment | Each plan’s credentialing status and effective date, in writing, with BlueCross BlueShield of Tennessee treated as its own payer |
| 2. Medicare and TennCare enrollment | The clinician’s Medicare record, and a TennCare Medicaid ID plus a contract with each TennCare plan you bill |
| 3. Member match | Name, date of birth, member ID and plan on the claim match the card on the date of service |
| 4. Claim setup | Billing and treating clinician names, NPIs and tax ID match each payer’s file; psychiatry add-on codes sit with the right E/M code |
| 5. Filing clocks | Medicare’s 12 months and each contract’s own window, written next to each payer’s name |
Ask what happens to visits before an effective date. Hold or reschedule those clients until the date is confirmed in writing.
Tennessee rules behind a Knoxville mental health billing service
Keep private, public and state rules apart. A claim follows the rules of its payer type.
Private payers
The footer of BlueCross BlueShield of Tennessee’s provider page (copyright 1998-2026) calls the company “an Independent Licensee of the Blue Cross Blue Shield Association.” Treat it as its own payer. Do not apply another state’s Blue plan rules to it, and do not merge it with other licensees.
The same page has a “Log in to Availity” link and a “Coverage & Claims” link under Tools & Resources. Your contract controls the rest, so read it with our payer contract compliance guide, and keep payer contacts in our insurance companies contact directory.
Medicare and Medicaid
The Centers for Medicare & Medicaid Services (CMS) lists Palmetto GBA, LLC as the A/B Medicare Administrative Contractor for Jurisdiction J, which processes fee-for-service Part A and Part B claims for Alabama, Georgia and Tennessee. The page shows the contract as “Fully Implemented,” with an award announced August 29, 2024.
Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies.
TennCare’s managed care organizations page lists Wellpoint (formerly known as Amerigroup), BlueCare, UnitedHealthcare Community Plan and TennCare Select, and places Knox County in its East group. If your clients are in Knox County, expect those plans.
TennCare’s provider page says providers must “enroll with TennCare to receive a Medicaid ID,” and that each managed care organization “creates their own contracts with providers” and processes its own claims. Ask each plan for its filing window in writing before you bill.
State rules
The Tennessee Department of Commerce and Insurance describes its Insurance Division as protecting consumers, and its page links to a complaint process. The department’s complaint page lists a “Provider Complaint Form” but does not explain who may use it or what it covers. Complaints about TennCare go to the TennCare Oversight Division, per that page.
Confirm the right route with the payer or your counsel before you file anything.
How do I choose a Knoxville mental health billing service for a BlueCross and TennCare caseload?
Choose a Knoxville mental health billing service that can show, for each clinician, which payers are active and from what date, and that bills BlueCross, commercial, Medicare and TennCare claims by the rules of each.
Ask for your clean claim rate, a list of denial reasons and a sample monthly report. Ask how the service reads remittances, since each denial code points to a fix. Use our guide to CARC and RARC codes to check its answers. Get fee terms in writing, and confirm in writing that the service bills in Tennessee.
What to do Monday morning
- List every clinician with each payer’s enrollment status and effective date.
- Add a plan type column to your client list: BlueCross, commercial, Original Medicare, Medicare Advantage or TennCare plan.
- Check that each clinician’s Medicare record is current before the next Medicare client visit.
- Confirm your TennCare Medicaid ID and which TennCare plans hold your contract.
- Write each payer’s filing window next to its name, using your contract.
- Send the five checks to each Knoxville mental health billing service you are considering.
The short version: Knoxville mental health billing service
A Knoxville mental health billing service earns the job by confirming enrollment before claims go out, treating BlueCross BlueShield of Tennessee as its own payer, routing Medicare and TennCare claims correctly and tracking every payer’s clock. Get the answers, reports and fees in writing.

FAQ
Does a billing service need to be in Knoxville?
No. Claims and reports move electronically, so a service can work with practices in Knoxville and across Tennessee. Tennessee payer knowledge, written terms and clear reporting matter more than an address. Ask the service to confirm in writing that it bills in Tennessee.
Is BlueCross BlueShield of Tennessee the same as other Blue plans?
No. Its provider page calls it an Independent Licensee of the Blue Cross Blue Shield Association. Use its own provider materials and your contract, not another state’s Blue plan rules.
Who processes Medicare claims in Tennessee?
CMS lists Palmetto GBA, LLC as the A/B Medicare Administrative Contractor for Jurisdiction J, which covers Alabama, Georgia and Tennessee. Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies.
Which plans does TennCare use?
TennCare’s managed care organizations page lists Wellpoint (formerly known as Amerigroup), BlueCare, UnitedHealthcare Community Plan and TennCare Select. Each plan contracts with providers and processes its own claims, so ask each one for its filing window in writing.
Do I need a Medicaid ID to bill TennCare?
TennCare’s provider page says providers who serve TennCare members must enroll with TennCare to receive a Medicaid ID. After that, you can contract with a managed care organization for claims payment.
