San Antonio Mental Health Billing Service: 5 Easy Checks

A San Antonio mental health billing service is an outside company that sends insurance claims, posts payments and follows up on denials for therapy and psychiatry practices in San Antonio, in Bexar County, Texas. It matters most when a group practice adds clinicians and takes Texas Medicaid and military-family clients alongside commercial plans. This guide gives five checks to run on every new clinician, plus the Texas rules behind them.

San Antonio and Bexar County: where a mixed caseload strains billing

The Texas State Historical Association’s Bexar County entry says San Antonio is the county seat and largest city. The U.S. Census Bureau’s QuickFacts page for San Antonio lists a median household income of $65,056 for 2020 to 2024 in 2024 dollars.

A median says nothing about one client’s plan. A San Antonio mental health billing service should look at each client’s card, not at the zip code.

Adding clinicians to a Medicaid, commercial and military-family caseload

Each new clinician brings a license, a National Provider Identifier (NPI) and a new set of payer enrollments. If claims go out before enrollment is finished, they can deny, and the denial can take weeks to untangle.

A San Antonio mental health billing service should track every clinician’s status with every payer in one place. It should also run insurance verification before each client’s first visit.

Out-of-network clinicians

If one of your clinicians is out of network with a plan, ask whether the service bills out-of-network claims. MCM South bills out-of-network claims for clients.

Five checks a San Antonio mental health billing service should run when you add a clinician

Work through these five checks in order for every new hire, and again when a payer changes its rules.

CheckWhat to confirm
1. Commercial enrollmentEach plan’s credentialing status and effective date, in writing
2. Medicare enrollmentThe clinician’s record and group link in the Medicare enrollment system
3. Texas Medicaid enrollmentEnrollment for the clinician’s license type, plus which STAR or STAR+PLUS plan each client uses
4. TRICARE clientsWhich TRICARE region and contractor handle each military-family client’s claims
5. Claim setup and filing clocksNames, NPIs and tax ID match each payer’s file; Texas managed care’s 95 days, Medicare’s 12 months, TRICARE’s one year and each contract’s own window

Ask what happens to visits before an effective date. Hold or reschedule those clients until the date is confirmed in writing.

Texas rules behind a San Antonio mental health billing service

Keep private, public and state rules apart. A claim follows the rules of its payer type.

Private payers

Blue Cross and Blue Shield of Texas signs its provider pages as a division of Health Care Service Corporation and an independent licensee of the Blue Cross and Blue Shield Association. Treat it as its own payer, and do not apply another state’s Blue plan rules to it.

Your contract sets each commercial plan’s filing window. Read it with our payer contract compliance guide, and keep payer contacts in our insurance companies contact directory.

For TRICARE, the military health program, the TRICARE regions page puts Texas in the West Region, with TriWest Healthcare Alliance as contractor. TRICARE’s claims deadline page gives one year from the date of service to file a medical claim in the United States.

Medicare and Medicaid

The Centers for Medicare & Medicaid Services (CMS) Jurisdiction H award fact sheet says Novitas Solutions, Inc. won the Jurisdiction H contract, which includes Texas. Medicare.gov sets a 12-month filing limit for Original Medicare.

The state’s managed care service areas map (revised January 2026) lists these plans in the Bexar service area. STAR: Aetna, Community First, Superior and Wellpoint. STAR+PLUS: Community First, Molina and United. So the first question for each Medicaid client is which plan to bill.

The Texas Medicaid and Healthcare Partnership (TMHP) managed care handbook says claims must reach the plan within 95 days of the date of service. If a claim went to the wrong plan on time, resubmit it to the correct plan with proof of the original timely filing, within 95 days of the date on the other carrier’s Remittance and Status Report. The plan must honor the original filing date.

TMHP’s behavioral health handbook says licensed clinical social workers, marriage and family therapists, professional counselors and psychologists must also be enrolled in Medicare or obtain a pediatric practice exemption. It limits psychotherapy codes 90832, 90833, 90834, 90836, 90837, 90838, 90846, 90847 and 90853 to 30 visits per calendar year, and more visits need prior authorization.

The TMHP telecommunication handbook (April 2026) says telehealth services must be billed with modifier 95, and that a client’s home may be the patient site.

State rules

The Texas Department of Insurance (TDI) prompt pay FAQ (last updated October 1, 2025) says a plan must pay an electronic clean claim by the 30th day, and a non-electronic clean claim by the 45th day. It also says a provider must file a claim within 95 days of the date of service, and that a provider and a plan may agree by contract to extend that period.

The same FAQ says the prompt pay laws do not apply to several plan types, including self-funded ERISA plans, Medicaid, Medicare Advantage and Tricare Standard. Ask during verification whether a plan is fully insured or self-funded.

On parity, TDI’s mental health parity page (December 15, 2025) says fully insured plans must cover mental health and substance use disorder benefits at the same level as physical illness benefits. It says the U.S. Department of Labor regulates self-funded plans.

If an insurer pays late or wrongly, TDI’s provider complaint page (April 28, 2025) lets providers and their billing services file online. You need the front of the patient’s ID card, the explanation of benefits (EOB), proof that appeals are exhausted and a timeline.

How do I choose a San Antonio mental health billing service as my group adds Medicaid clients?

Choose a San Antonio mental health billing service that can show, for each clinician, which payers are active and from what date, and that bills Medicaid, TRICARE and commercial claims by the rules of each.

Ask for your clean claim rate, a list of denial reasons and a sample monthly report. Ask how the service reads remittances, since each denial code points to a fix. Use our guide to CARC and RARC codes to check its answers. Get fee terms in writing, and confirm in writing that the service bills in Texas.

What to do Monday morning

  1. List every clinician with each payer’s enrollment status and effective date.
  2. Mark which clients are on STAR, STAR+PLUS, TRICARE, Medicare or commercial plans.
  3. Check how each plan is funded, fully insured or self-funded, and note it in the chart.
  4. Pull your last remittance and note the top three denial reasons.
  5. Write down your next hire’s start date and work backward through the five checks.
  6. Send the five checks to each San Antonio mental health billing service you are considering.

The short version: San Antonio mental health billing service

A San Antonio mental health billing service earns the job by getting every clinician enrolled before claims go out, routing Medicaid, TRICARE and commercial claims by the rules of each, and tracking every payer’s clock. Get the answers, reports and fees in writing.

FAQ

Does a billing service need to be in San Antonio?

No. Claims and reports move electronically, so a service can work with practices in San Antonio, elsewhere in Bexar County and across Texas. Texas payer knowledge, written terms and clear reporting matter more than an address. Ask the service to confirm in writing that it bills in Texas.

How long do I have to bill a Texas Medicaid managed care plan?

TMHP’s managed care handbook says claims must reach the plan within 95 days of the date of service. A claim sent to the wrong plan on time can be resubmitted to the correct plan with proof, within 95 days of the other carrier’s Remittance and Status Report date.

Which Medicaid plans serve Bexar County?

The state’s January 2026 service areas map lists Aetna, Community First, Superior and Wellpoint for STAR, and Community First, Molina and United for STAR+PLUS. Verify each client’s plan on the date of service.

Is Blue Cross and Blue Shield of Texas the same as other Blue plans?

No. Its provider pages call it a division of Health Care Service Corporation and an independent licensee of the Blue Cross and Blue Shield Association. Use its own provider materials and your contract, not another state’s Blue plan rules.

Where do I report a Texas insurer that pays late?

TDI’s provider complaint page lets providers and their billing services file online. It says the complaint goes to TDI when the patient’s ID card says TDI or DOI. Keep the ID card copy, EOBs, appeal records and a timeline ready.
