South Texas Mental Health Billing Service: 5 Easy Essentials

A South Texas mental health billing service handles insurance claims for therapy and psychiatry practices in the region from San Antonio to the Rio Grande Valley and the Gulf coast. Texas does not use one Medicaid plan list for the whole region. It splits South Texas into service areas, and each area has its own health plans. This guide shows the split and five essentials to confirm before you outsource.

What a South Texas mental health billing service has to sort out first

A South Texas mental health billing service has to know which Medicaid service area each client lives in, because Texas assigns health plans by area. The Texas Health and Human Services Commission (HHSC) groups South Texas counties into the Bexar, Hidalgo and Nueces service areas. The client’s plan on the date of service decides where the claim goes.

Service areaCounties named by HHSCSTAR plans (children, families)STAR+PLUS plans (adults 65+, adults with disabilities)
BexarBexar, Comal, Guadalupe, Kendall, Medina and three othersAetna, Community First, Superior, WellpointCommunity First, Molina, United
HidalgoHidalgo, Cameron, Webb, Starr, Maverick and five othersDriscoll, Molina, Superior, UnitedSuperior, Molina, United
NuecesNueces, Victoria, Kleberg, San Patricio and ten othersDriscoll, Superior, UnitedSuperior, Wellpoint

Counties come from an HHSC handbook appendix (revision 25-2, effective June 6, 2025). Plans come from HHSC’s service area map, revised January 2026. Laredo’s county, Webb, sits in the Hidalgo area, and Corpus Christi’s county, Nueces, anchors the Nueces area. Community First Health Plans is based in San Antonio and was established by University Health. Driscoll Health Plan is based in Corpus Christi and serves 24 South Texas counties.

Rules by payer type for a South Texas mental health billing service

Keep private, Medicare and Medicaid rules separate. Each has its own filing clock and its own appeal path.

Private payers

Blue Cross and Blue Shield of Texas is a division of Health Care Service Corporation and an independent licensee of the Blue Cross and Blue Shield Association, per its provider site. Use its Availity link for claims and eligibility. Names such as Aetna and United on HHSC’s map are Medicaid plan listings. Their commercial plans follow separate manuals, so do not assume one rule covers both.

Medicare and Medicaid

The Centers for Medicare & Medicaid Services (CMS) lists Texas in Jurisdiction H, where Novitas Solutions holds the Original Medicare contract (announced June 24, 2026). For Medicaid, the Texas Medicaid & Healthcare Partnership (TMHP) managed care handbook says an initial claim sent to the correct plan must arrive within 95 days of the date of service. The behavioral health handbook says counselors, marriage and family therapists, clinical social workers and psychologists must be enrolled in Medicare or hold a pediatric practice exemption. It also limits listed psychotherapy codes to 30 visits per calendar year, with more needing prior authorization. A plan may apply its own steps. A Blue Cross Medicaid notice (5/14/2024) says that for LPC and LMFT claims, Medicare must be billed first or Texas Medicaid will deny them.

State rules

The Texas Department of Insurance (TDI) prompt pay FAQ says fully insured plans owe payment on clean claims within 30 days (electronic) or 45 days (paper), and providers file within 95 days. Medicaid and self-funded plans are excluded. For telehealth, Texas Medicaid pays audiovisual visits with modifier 95 and audio-only behavioral health with FQ, per the April 2026 handbook. Under Texas Insurance Code 1455.004, plans cover telehealth on the same basis as in person.

What to do Monday morning

  1. Add a county column to your client list.
  2. Match each Medicaid client’s county to its service area in the table above.

  5. Ask any service you consider for a sample monthly paysheet report.
