Private payer: claims

File through Availity. Service Facility NPI is required on professional claims (loop 2310C on the 837P, or boxes 32/32a/32b on the CMS-1500), except for Medicare crossover and Medicaid. Missing it causes rejection code AP0677 (Florida Blue provider notice above). Florida Blue's own provider pages do not print a payer ID or filing limit, so use your Availity enrollment and your contract.

Private payer: appeals

Start with a Provider Reconsideration, then an Administrative Appeal. Submit within one year of the remittance advice date (instructions form).

Private payer: behavioral health

Florida Blue's quick reference guide lists Lucet for psychiatric and substance use needs at (866) 730-5006. A 2023 notice says myBlue members are authorized through Lucet for inpatient, partial hospitalization, intensive outpatient and substance use rehab. Confirm the scope for other products with Florida Blue (Lucet notice). Florida Blue's behavioral health page lists virtual treatment (page).
