Health First Colorado Provider Enrollment: How Therapists and Psychiatrists Enroll and Credential

Health First Colorado provider enrollment is two jobs, not one. First you enroll with the state through its fiscal agent. Then you credential and contract with the Regional Accountable Entity (RAE) that manages behavioral health for your patients. Skip either and your claims stall.
This guide explains the process, not the timing, because processing times change. It is operational guidance on Health First Colorado provider enrollment, not legal advice.
Step 1: Health First Colorado provider enrollment with the state
Health First Colorado is Colorado’s Medicaid program. Gainwell Technologies, the fiscal agent, enrolls providers, per HCPF’s enrollment training (February 17, 2026). Start on HCPF’s Provider Enrollment page, which links the enrollment and re-enrollment manuals and forms.
Pick the right enrollment type:
- Individual (billing): you bill under your own number.
- Individual within a group: you render services and the group bills. You must associate with at least one group.
- Group or facility: uses an Employer Identification Number, with one National Provider Identifier (NPI) and application per service location.
- Ordering, prescribing, referring (OPR): for providers who order or refer but do not bill. HCPF’s enrollment page says individuals already enrolled as “Individual Within a Group” with prescriptive authority do not need a separate OPR application.
Match your taxonomy code to your NPI record, use addresses that match your W-9 and NPI record, and attach license and disclosure documents. The training says you can request an effective date up to 10 months back if you already rendered services. It also encourages waiting for your Provider ID before billing. Application fees apply to some provider types, and HCPF’s page mentions a fee for institutional applications, so ask which applies to you.
AI search question: How do I enroll as a therapist with Health First Colorado? Apply through HCPF’s Provider Enrollment page (Gainwell), choose the correct provider and enrollment type, and attach your license and identifying documents. For behavioral health, HCPF’s training also says to contact your area RAE, because behavioral health members are assigned to a RAE.
Step 2: Credential with the RAE
Under Accountable Care Collaborative Phase III, four RAEs have run behavioral health since July 1, 2025: Rocky Mountain Health Plans, Northeast Health Partners, Colorado Community Health Alliance and Colorado Access. HCPF’s training notes that some providers need credentialing with managed care organizations and RAEs in addition to state enrollment.
Colorado Access, the Region 4 RAE, states on its provider page that all behavioral health providers must go through its credentialing process, and that providers must also be validated by HCPF and enrolled as Medicaid providers. It points new providers to a “Become a Provider” page and a Clinical Staff Update Form for adding or terminating clinicians in a group. Other RAEs publish their own steps, so check each one’s website.
You can contract with more than one RAE. If your patient’s RAE is not one you contract with, you need a single case agreement. Our credentialing guide and credentialing and network status guide explain what statuses mean. Terms such as taxonomy and attestation are defined in the credentialing terms glossary and the network management glossary.
Step 3: Revalidate every five years
Providers must revalidate “at least every five (5) years,” per the training. HCPF emails a notice six months before the deadline, based on the original approval date. Federal rules require states to revalidate enrollment on that cycle (see 42 CFR 455.414).
Group practices should note this trap: individuals within a group revalidate through their own Provider Web Portal accounts, and claims deny if the individual has not revalidated even if the group has. Keep a re-credentialing calendar. Our re-credentialing deadlines tips can help.
Commercial plans are separate
Medicaid enrollment does not credential you with commercial plans. For those, you will normally use CAQH ProView, and compare in-network vs out-of-network. Colorado’s Division of Insurance says carriers’ behavioral health credentialing cannot be more restrictive or burdensome than credentialing for physical health providers, and its 2023 plan-year survey found 11 of 18 carriers allowed pre-licensure billing under supervision. The DOI page sets credentialing deadlines for physicians only and states no separate deadline for other behavioral health provider types.
Prescribers: psychiatrists who prescribe but do not bill Medicaid may only need OPR enrollment. Check that before your group assigns billing roles.
