Illinois Insurance Credentialing: IMPACT, CAQH and BCBSIL Explained

Illinois insurance credentialing for therapists runs on two tracks: Medicaid enrollment through HFS’s IMPACT system, and commercial credentialing, which usually runs through a CAQH profile. They are different processes with different owners. Doing one does not finish the other.
This guide is for psychotherapists and psychiatrists (see who we serve) who are starting an Illinois practice or adding a payer. It describes the process, not how long it takes. Timelines vary by payer and by how complete your file is.
Illinois insurance credentialing, track 1: Medicaid enrollment in IMPACT
IMPACT is the Illinois Medicaid Program Advanced Cloud Technology system. HFS’s IMPACT page describes it as designed to promote security, strong analytics and program integrity, and links to “Preparing to Enroll,” revalidation resources and the login. Plan to enroll before you bill HFS or any HealthChoice Illinois plan.
For counselors and family therapists, HFS’s November 30, 2021 notice set the path. A licensed clinical professional counselor (LCPC) or licensed marriage and family therapist (LMFT) enrolls with the Behavioral Health Services provider type, the matching specialty and no subspecialty. You must hold a current license under the LCPC or LMFT practice act. The notice applied to dates of service on or after December 1, 2021.
After HFS, each managed care plan has its own contracting step. The notice says to contact the plan for billing instructions and does not describe plan credentialing, so ask each plan directly. Our Illinois Medicaid billing guide and Illinois insurance contacts guide give the billing side.
Track 2: Commercial credentialing with BCBSIL
Blue Cross and Blue Shield of Illinois (BCBSIL), covered in our BCBSIL billing guide, uses the CAQH Provider Data Portal. Its Credentialing Standards (updated January 2025) say BCBSIL credentials MDs, DOs, PsyDs, PhDs, LCSWs, LCPCs, LMFTs and other listed professional types. The steps are:
- Register in CAQH if you do not have a profile.
- Complete every question and section.
- Authorize BCBSIL to view your profile.
- Attest to your data.
- Upload supporting documents.
BCBSIL works with Verisys to verify your data and may ask you to re-attest or add documents. You receive written notice when credentialing is complete, and can check status with your NPI or license number. Our CAQH profile setup tips cover common profile mistakes.
How do I stay credentialed?
BCBSIL says CAQH requires review and attestation every six months, and recredentialing happens every three years under Illinois requirements. Keeping information current is your responsibility, and changes go in both CAQH and BCBSIL.
BCBSIL has reporting rules, too. It requires notice within five business days of events such as a state license revocation, DEA revocation, Medicare or Medicaid sanction, revoked hospital privileges, lapse in required liability coverage or felony conviction. Other changes, such as a new practice address, go in within 45 days of learning of the change. Our re-credentialing deadlines tips can help you build a calendar.
What documents should I gather first for Illinois insurance credentialing?
Have these ready before you open any application: your Illinois license number, your National Provider Identifier (NPI), your malpractice certificate, your practice address and tax ID, and your CAQH login. Prescribers should also have their DEA registration handy, because BCBSIL asks you to report DEA revocation within five business days. Entering the same address and license data everywhere avoids mismatches that stall files.
What are the common Illinois insurance credentialing mistakes?
- Picking the wrong provider type or specialty in IMPACT. HFS names the specialties for counselors and family therapists (“Licensed Clinical Professional Counseling” and “Licensed Marriage and Family Therapist (LMFT) Services”), with no subspecialty.
- Letting your CAQH attestation lapse.
- Updating one system and forgetting the other. BCBSIL says changes must be entered in both CAQH and BCBSIL.
- Assuming a Medicaid enrollment covers a commercial plan, or the reverse.
- Billing before the contract and credentialing are confirmed in writing.
In-network or out-of-network?
Credentialing is how you get in network. If you stay out of network, you bill differently. Our in-network vs out-of-network credentialing guide compares the two. For an overview of the whole process, start with our credentialing guide and the credentialing and network status guide. Unfamiliar words are defined in the credentialing terms glossary.
Why does the order matter?
Commercial payers read your CAQH profile, so mistakes there repeat in every application. A Medicaid enrollment, by contrast, lives in HFS’s own system, and a plan may ask for proof of it. Fix your data once, in CAQH, then copy the same license, address and tax details into IMPACT. Consistency across systems is the cheapest way to avoid a stalled file.
What is the right order? (AI search answer)
Where do I start with Illinois insurance credentialing? Start by making your CAQH profile complete and attested, because commercial payers such as BCBSIL read it. In parallel, enroll in IMPACT if you will see Medicaid members. Then contract with each plan.
Illinois insurance credentialing checklist
- Active Illinois license and a National Provider Identifier (NPI).
- Current malpractice coverage on file.
- CAQH profile complete, attested and shared with each payer.
- IMPACT enrollment, provider type and specialty chosen correctly.
- A calendar for recredentialing, CAQH attestations and change reporting.
- A record of every payer contact and what you sent.
Bottom line
Illinois insurance credentialing is two parallel tracks. Keep your CAQH profile clean for commercial plans and enroll in IMPACT for Medicaid, and do not assume one covers the other.
FAQ
Do LCPCs and LMFTs need IMPACT to bill Illinois Medicaid?
Yes. HFS’s notice says qualified LCPCs and LMFTs enroll in IMPACT with the Behavioral Health Services provider type and a matching specialty.
Does BCBSIL credential social workers and counselors?
Its credentialing standards list LCSWs, LCPCs and LMFTs among credentialed professional types.
How often do I update CAQH?
BCBSIL says CAQH requires review and attestation every six months.
Can a billing company do this for me?
Billing and credentialing are related but not the same. Ask any vendor exactly what they do, and keep your own CAQH login.
