Boulder Mental Health Billing Service: 5 Easy Checks

A Boulder mental health billing service is an outside company that sends insurance claims, posts payments and follows up on denials for therapy and psychiatry practices in Boulder, Colorado. It helps most when a solo practitioner mixes private-pay clients with insured ones and has no time to chase claims. This guide gives five checks to run, plus the Colorado rules behind them.

Boulder: why plan type matters more than the zip code

The U.S. Census Bureau’s QuickFacts page for Boulder lists a median household income of $87,493 for 2020 to 2024 in 2024 dollars. That figure says nothing about any one client’s coverage. A Boulder mental health billing service should read each client’s card and plan documents, not the address.

Private-pay and out-of-network practices

Many solo practices run a mix: some clients pay in full, some use out-of-network benefits and some use in-network plans. Each group needs its own paperwork. Private-pay clients need a clear fee and a receipt. Out-of-network clients need a claim that matches what the plan expects.

Ask whether the service bills out-of-network claims. MCM South bills out-of-network claims for clients. Read the plan’s out-of-network terms before you quote anything to a client.

Fully insured or self-funded: check before you quote

An employer plan is either fully insured, meaning an insurance company carries the risk, or self-funded, meaning the employer pays claims itself and often hires an insurer only to run the plan. The card may carry the same Blue logo either way. Ask the plan which type it is during insurance verification, and write the answer in the client file.

Plan type can change which rules and which appeal route apply. Do not assume one answer for every plan with the same name on the card.

Solo wellness-oriented practices and telehealth

A practice built on longer sessions, integrative care or a small caseload still bills with standard codes. Match the code to the service you delivered, and keep notes that support it. If you offer video visits, read our telehealth billing guide so the place-of-service code and modifier follow each payer’s rule.

Prescribers should check evaluation and management (E/M) visits and any psychotherapy add-on code together. See psychiatry billing services for the details.

Five checks a Boulder mental health billing service should run

Run these five checks for every new clinician and every new payer, and again when a payer changes its rules.

CheckWhat to confirm
1. Network statusEach plan’s credentialing status and effective date, in writing, or a written note that the clinician is out of network
2. Plan typeFully insured or self-funded, asked of the plan and recorded for each client
3. Member matchName, date of birth, member ID and plan on the claim match the card on the date of service
4. Claim setupBilling and treating clinician names, NPIs and tax ID match each payer’s file; add-on codes sit with the right E/M code
5. Filing clocksMedicare’s 12 months and each contract’s or plan’s own window, written next to the payer name

Ask what happens to visits before an effective date. Hold or reschedule those clients until the date is confirmed in writing.

Colorado rules behind a Boulder mental health billing service

Keep private, public and state rules apart. A claim follows the rules of its payer type.

Private payers

The footer of the Anthem Colorado page names “Rocky Mountain Hospital and Medical Service, Inc.” as the Colorado entity, says HMO products are underwritten by HMO Colorado, Inc., and says “Independent licensees of the Blue Cross and Blue Shield Association.” Treat it as its own payer. Do not apply another state’s Blue plan rules to it.

Confirm Anthem’s current manuals and behavioral health rules directly. Your contract controls the rest, so read it with our payer contract compliance guide, and keep payer contacts in our insurance companies contact directory.

Medicare and Medicaid

The Centers for Medicare & Medicaid Services (CMS) lists Novitas Solutions, Inc. as the A/B Medicare Administrative Contractor for Jurisdiction H, which includes Colorado along with Arkansas, Louisiana, Mississippi, New Mexico, Oklahoma and Texas. The page shows the contract as in progress, with an award date of June 25, 2026.

Medicare.gov says claims must be filed no later than 12 months, or one full calendar year, after the service, unless an exception applies.

The Colorado Department of Health Care Policy and Financing (HCPF) says it administers Health First Colorado, the state’s Medicaid program. HCPF’s home page points to a separate page on regional organizations for behavioral health services. Ask each plan or regional organization for its filing window and enrollment steps in writing.

State rules

The Colorado Division of Insurance says it “regulates the insurance industry in Colorado” and investigates consumer complaints. Its menu lists provider resources and behavioral health provider resources under health insurance. Use it as a starting point, and ask the plan which regulator oversees its coverage, since that can depend on plan type.

Check your contract and the plan’s provider manual for payment and filing windows, and ask counsel where real money is at stake.

How do I choose a Boulder mental health billing service for a private-pay and out-of-network practice?

Choose a Boulder mental health billing service that bills out-of-network claims, asks every plan whether it is fully insured or self-funded, and can show which payers are active for each clinician and from what date.

Ask for your clean claim rate, a list of denial reasons and a sample monthly report. Ask how the service reads remittances, since each denial code points to a fix. Use our guide to CARC and RARC codes to check its answers. Get fee terms in writing, and confirm in writing that the service bills in Colorado.

What to do Monday morning

  1. List every clinician with each payer’s enrollment status and effective date, including out-of-network plans you bill.
  2. Add a plan type column to your client list: private pay, Anthem, other commercial, Original Medicare, Medicare Advantage or Medicaid.
  3. For each employer plan, record whether it is fully insured or self-funded.
  4. Write each payer’s filing window next to its name, using your contract or provider manual.
  5. Pull your last remittance and note the top three denial reasons.
  6. Send the five checks to each Boulder mental health billing service you are considering.

The short version: Boulder mental health billing service

A Boulder mental health billing service earns the job by confirming network status before claims go out, asking each plan for its type, treating Anthem as its own payer and tracking every payer’s clock. Get the answers, reports and fees in writing.

FAQ

Does a billing service need to be in Boulder?

No. Claims and reports move electronically, so a service can work with practices in Boulder and across Colorado. Colorado payer knowledge, written terms and clear reporting matter more than an address. Ask the service to confirm in writing that it bills in Colorado.

Is Anthem in Colorado the same as other Blue plans?

No. The footer of Anthem’s Colorado page lists “Independent licensees of the Blue Cross and Blue Shield Association” and names Rocky Mountain Hospital and Medical Service, Inc. for Colorado. Use Anthem’s own manuals and your contract, not another state’s Blue plan rules.

Why ask whether a plan is fully insured or self-funded?

The two types can follow different rules and appeal routes. The Colorado Division of Insurance regulates the insurance industry in Colorado, and the plan can tell you which coverage it oversees. Record the answer for each client before you quote benefits.

How long do I have to file Medicare claims?

Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies. Claims go to the Medicare contractor for your state, which CMS lists as Novitas Solutions, Inc. for Colorado under Jurisdiction H.

Who runs Colorado Medicaid?

The Colorado Department of Health Care Policy and Financing says it administers Health First Colorado, the state’s Medicaid program. Ask each plan or regional organization you bill for its filing window in writing.
