BCBSTX E/M Downcoding: What Psychiatrists Should Do Now
BCBSTX E/M downcoding is Blue Cross and Blue Shield of Texas paying a lower evaluation and management (E/M) level when its review finds the note does not support the level you billed. The Texas Medical Association (TMA) says it applies to commercial claims with dates of service on or after July 1, 2026. Psychiatrists who bill E/M visits should check their remittances now.
What changed in BCBSTX E/M downcoding?
TMA reports (published May 22, 2026, updated August 24, 2026) that BCBSTX will check whether the services support the E/M level on a claim for office, inpatient and outpatient visits. If they do not, BCBSTX pays the lower level. BCBSTX says its review follows American Medical Association (AMA) guidelines for level of service and medical decision-making (MDM), and that a physician who disagrees can submit medical records.
Sister Blue plans that use the same claim-editing change published matching notices on March 16, 2026. The Oklahoma notice says claims are paid at the lower level the services support, that no action is needed if you agree, and that you should send records if you disagree. BCBSTX’s policy list shows the related policy as CPCP024, “Evaluation and Management Coding – Professional Provider Services.”
When it takes effect
The claim editing applies to dates of service on or after July 1, 2026, per TMA and the Federation of Texas Psychiatry (August 18, 2026). BCBSTX’s policy list shows an effective date of February 23, 2026 for CPCP024, posted November 21, 2025. The two dates differ, so ask BCBSTX which one governs your claims.
Who it affects
TMA describes the change for commercial BCBSTX plans. Texas Medicaid STAR is a separate program; see our Texas Medicaid STAR billing guide. Psychiatrists who bill E/M visits are the group to watch. Psychotherapy add-on codes 90833, 90836 and 90838 are recognized when billed with a separately identifiable E/M service, and the E/M is the primary code that can be reviewed. The Federation of Texas Psychiatry says BCBSTX has not said the add-on codes themselves are downcoded. Stand-alone psychotherapy codes such as 90834 and 90837 are not E/M codes, and the BCBSTX notices do not name them.
Private payers

Medicare and Medicaid
The BCBSTX notices describe commercial plans only. Nothing in them extends the change to Medicaid or Medicare Advantage, so do not treat it as a rule for those programs.
State rules
BCBSTX E/M downcoding is part of a multi-state program run by the Blue plans owned by Health Care Service Corporation (HCSC), according to the Federation of Texas Psychiatry. Illinois is also affected.
A September 21, 2026 law firm article says Blue Cross and Blue Shield of Illinois began the same kind of review on July 1, and that Illinois’s Transparency in Downcoding Act takes effect January 1, 2028. The article says the Act defines health care professionals as physicians, physician assistants and advanced practice registered nurses, so some behavioral health professionals may fall outside it.
A Chicago Sun-Times report (August 26, 2026) says mental health clinicians are among those affected.

Practical impact
Downcoded claims may pay less without being denied, so BCBSTX E/M downcoding can hide in the remittance. The Federation of Texas Psychiatry, citing AMA advice, tells practices to compare what was paid with what they expected, because the billed code may not visibly change. What do I do if BCBSTX downcodes my E/M claim?
Compare the payment to your expected allowed amount. If your note supports the level you billed, send the records through the process BCBSTX gives you and keep a copy. If the note does not support it, fix the template, because BCBSTX E/M downcoding will repeat. Log every case so you can see the pattern.
