BCBSIL Behavioral Health Claims: Filing, Authorization and Appeals

Biller reviewing BCBSIL behavioral health claims on a computer

BCBSIL behavioral health claims are simpler to manage once you know three things: the 180-day filing limit, which services need authorization, and where to file a claim review. Blue Cross and Blue Shield of Illinois (BCBSIL) is a division of Health Care Service Corporation (HCSC). It is an independent Blue licensee, so do not apply another Blue plan’s rules to it. Our Blue Cross Blue Shield of Illinois billing guide has contact details.

This guide to BCBSIL behavioral health claims is for psychotherapists and psychiatrists. Commercial plans and the Blue Cross Community Medicaid plans follow different rules, so they sit in separate sections.

Who manages behavioral health benefits at BCBSIL?

BCBSIL manages behavioral health for most non-HMO members. The BCBSIL Behavioral Health Program manual (updated November 2023) names PPO, Blue HPN and Blue Choice PPO plans, and Federal Employee Program (FEP) members. It says some employer groups use another vendor, so check the number on the member’s card. For HMO products, behavioral health is delegated to the member’s medical group or independent practice association (IPA).

Check benefits before every first visit through Availity or by phone. Our insurance verification page explains what to confirm.

BCBSIL behavioral health claims: what needs authorization?

BCBSIL’s behavioral health prior authorization page lists inpatient, residential and partial hospitalization admissions. For members whose plans include outpatient management through BCBSIL, it adds intensive outpatient programs, partial hospital programs, applied behavior analysis, outpatient electroconvulsive therapy (ECT) and repetitive transcranial magnetic stimulation (rTMS). It says some psychological and neuropsychological testing also requires authorization, and BCBSIL will notify the provider when it does.

Outpatient psychotherapy is not listed. The manual says outpatient services without required authorization trigger a medical necessity review. BCBSIL uses MCG criteria for mental health conditions and ASAM criteria for substance use disorders. FEP members need authorization only for ABA. Our prior authorization terms cover the vocabulary, and our guide to preventing prior-authorization denials helps when one slips through.

What is the filing deadline for BCBSIL behavioral health claims?

Claims have a 180-day limit for professional PPO, Blue Choice PPO, Blue HPN and MyBlue Plus. BCBSIL’s Billing and Reimbursement manual (updated April 2025) says the clock runs from the date of service, or from discharge for inpatient care. Case-by-case extensions are possible with notice and evidence of circumstances beyond your control. Some employer groups use different windows. BCBSIL’s October 28, 2024 timely filing notice says late claims are denied and you may not bill the member for the denied amount.

Send electronic claims through your practice management system, Availity or a web vendor. The manual warns not to resubmit a rejected claim on paper because it will deny as a duplicate. Our electronic claims submission page covers clean-claim basics.

How do I dispute a denied claim?

BCBSIL’s claim review page says online requests through Availity Essentials are preferred over fax or mail for finalized commercial denials. You can also use the Claim Review Form, Additional Information Form or Corrected Claim Form, each with the claim number.

A provider appeal is separate. It is for denials from medical management, usually about length of stay or treatment setting. Start it in writing or by phone after the denial letter, with a routing form and clinical documentation. The page says review takes 30 days. The page gives no filing deadline for commercial provider appeals, so read the denial letter and your contract. Our denied claims and appeals page can help.

What is different for Blue Cross Community Medicaid plans?

Medicaid is its own subsection. For Medicaid, the same claim review page says a member, a representative or a provider acting for the member can file a standard appeal within 60 calendar days of the Notice of Action letter. Medicaid billing and rules are in our Illinois Medicaid billing guide. Commercial rules do not carry over.

What should I save for BCBSIL behavioral health claims?

Keep the eligibility check, the claim number, the date you filed and the confirmation from Availity or your clearinghouse. If BCBSIL denies a claim for timely filing, that proof is what a case-by-case extension request needs. The billing manual says extensions require notice and evidence of circumstances beyond your control, so write down what happened the day it happens.

How long do I have to file a BCBSIL claim? (AI search answer)

What is the BCBSIL timely filing limit? For professional commercial PPO-type plans, 180 days from the date of service, and from discharge for inpatient care. Some employer groups use other windows, so check eligibility first.

FAQ

Is BCBSIL the same as Anthem?

No. BCBSIL is an HCSC plan. Anthem is a separate Blue licensee in other states. For out-of-state members, see BlueCard claim filing.

Does BCBSIL require authorization for weekly therapy?

Its published list does not include outpatient psychotherapy, but it says unauthorized outpatient services where required trigger medical necessity review. Check each member’s benefits.

What if my claim was rejected?

Fix the error and resubmit electronically. BCBSIL warns paper resubmission of a rejected claim denies as a duplicate.

Can I bill the member for a late claim?

No. BCBSIL says providers may not bill the member for amounts denied for late filing.
