Asheville Mental Health Billing Service: 5 Easy Checks

An Asheville mental health billing service is an outside company that sends insurance claims, posts payments and follows up on denials for therapy and psychiatry practices in Asheville, North Carolina. It helps most when a solo practice mixes private-pay, out-of-network and in-network clients and sees some clients by video. This guide gives five checks to run, plus the North Carolina sources behind them.

Asheville: why western North Carolina practices need a clear payer map

The U.S. Census Bureau’s QuickFacts page for Asheville lists a median household income of $71,102 for 2020 to 2024 in 2024 dollars. That number does not tell you how any one client pays. An Asheville mental health billing service should read the card and the intake form, not the zip code.

Private-pay and out-of-network practices

Many solo clinicians keep part of the caseload private-pay or out-of-network. That is a sound choice, but it creates two kinds of paperwork. Some clients pay you and send their own claim. Others expect you to file for them.

Decide this per client at intake and write it down. If you want help with the second group, ask whether the service bills out-of-network claims. MCM South bills out-of-network claims for clients.

Video visits for clients outside town

A video visit can extend a practice to clients who live a long drive from your office. It also adds a billing question. The claim has to show how and where the visit happened in the way each payer asks.

Read the payer’s own telehealth rule, then use our telehealth billing guide to match the place-of-service code and modifier. Run insurance verification before the first video visit too.

A Medicare mix and psychiatry visits

Ask at intake whether a client has Original Medicare, a Medicare Advantage plan or retiree coverage. Each sends the claim to a different place. Record the plan name, member ID and the date coverage began.

Psychiatrists bill evaluation and management (E/M) visits, often with a psychotherapy add-on code on the same visit. Our guide to psychiatry billing services covers the pairing.

Five checks an Asheville mental health billing service should run

Run these five checks for every new clinician and every new payer, and again when a payer changes its rules.

CheckWhat to confirm
1. Blue Cross NC and commercial enrollmentEach plan’s credentialing status and effective date, in writing, with Blue Cross NC treated as its own payer
2. Medicare and Medicaid enrollmentThe clinician’s Medicare record, and North Carolina Medicaid enrollment where you take Medicaid clients
3. Network status by clientWhich clients are in network, out of network or private-pay, and who files the claim for each
4. Claim setupNames, NPIs and tax ID match each payer’s file; video-visit place of service and modifier follow the payer’s rule
5. Filing clocksMedicare’s 12 months and each contract’s own window, written next to each payer

Ask what happens to visits before an effective date. Hold or reschedule those clients until the date is confirmed in writing.

North Carolina rules behind an Asheville mental health billing service

Keep private, public and state sources apart. A claim follows the rules of its payer type.

Private payers

Blue Cross and Blue Shield of North Carolina (Blue Cross NC) is its own payer. Do not apply another state’s Blue plan rules to it. Its individual practitioner page (last modified August 10, 2026) lists four steps for professionals such as therapists with a Type 1 National Provider Identifier (NPI): register with CAQH and keep the profile current, submit the enrollment form, register for the Blue e portal and sign up for electronic funds transfer, then receive a Network Participation Agreement.

Blue Cross NC describes Blue e as the place to “Submit professional (CMS 1500) and institutional (UB04) claims electronically,” and says access is limited to registered, credentialed providers. Your contract controls the rest, so read it with our payer contract compliance guide. Keep payer contacts in our insurance companies contact directory.

Medicare and Medicaid

The Centers for Medicare & Medicaid Services (CMS) lists Palmetto GBA, LLC as the A/B Medicare Administrative Contractor for Jurisdiction M. The page lists North Carolina, South Carolina, Virginia and West Virginia for Part A and Part B and shows the contract as “Fully Implemented.”

Medicare.gov says claims must be filed no later than 12 months after the service, with possible exceptions.

For North Carolina Medicaid, the state’s NC Medicaid site links a Provider Playbook for Medicaid managed care, Health Plan Billing Guidance, Provider Enrollment and a Behavioral Health and Intellectual/Developmental Disabilities Tailored Plan page. Start there. Ask each health plan for its claim filing window in writing.

State rules

The North Carolina Department of Insurance (NCDOI) home page says the department helps North Carolinians with insurance, including health coverage. Its health claim denied page describes Smart NC as a program for consumers, not providers.

Confirm filing and payment deadlines in your contract or with counsel.

How do I choose an Asheville mental health billing service for a mixed private-pay and insurance practice?

Choose an Asheville mental health billing service that can show, for each clinician, which payers are active and from what date, and that handles in-network, out-of-network and Medicare claims by the rules of each.

Ask for your clean claim rate, a list of denial reasons and a sample monthly report. Ask how the service reads remittances, since each denial code points to a fix. Use our guide to CARC and RARC codes to check its answers. Get fee terms in writing, and confirm in writing that the service bills in North Carolina.

What to do Monday morning

  1. List every clinician with each payer’s enrollment status and effective date.
  2. Mark each client as in network, out of network or private-pay, and note who files the claim.
  3. Check that each clinician’s Medicare record is current before the next Medicare client visit.
  4. Write each payer’s filing window next to its name, using your contract.
  5. Read the payer’s video-visit rule and compare it with your last three telehealth claims.
  6. Send the five checks to each Asheville mental health billing service you are considering.

The short version: Asheville mental health billing service

An Asheville mental health billing service earns the job by confirming enrollment before claims go out, treating Blue Cross NC as its own payer, keeping out-of-network and private-pay clients straight and tracking every payer’s clock. Get the answers, reports and fees in writing.

FAQ

Does a billing service need to be in Asheville?

No. Claims and reports move electronically, so a service can work with practices in Asheville and across North Carolina. North Carolina payer knowledge, written terms and clear reporting matter more than an address. Ask the service to confirm in writing that it bills in North Carolina.

Is Blue Cross NC the same as Anthem or other Blue plans?

No. Blue Cross NC is its own payer with its own provider pages and portal. Use its manuals and your contract, not another state’s Blue plan rules.

Who processes Medicare claims for North Carolina?

CMS lists Palmetto GBA, LLC as the A/B Medicare Administrative Contractor for Jurisdiction M, which includes North Carolina. Medicare.gov says claims must be filed no later than 12 months after the service, with possible exceptions.

Where do I find North Carolina Medicaid billing guidance?

The NC Medicaid site links a Provider Playbook for Medicaid managed care, Health Plan Billing Guidance and Provider Enrollment pages. Ask each health plan for its claim filing window in writing before you bill.

Can I bill video visits for clients who live far from Asheville?

It depends on the payer. Read each payer’s own telehealth rule and match the place-of-service code and modifier to it. Confirm with the payer in writing before you bill, and see our telehealth billing guide for the basics.
