Clinician with a patient in a session for CPT code 90839 crisis psychotherapy billing

CPT 90839 vs 90834 and 90837: When Crisis Psychotherapy Applies

CPT code 90839 is the code for psychotherapy for crisis. It is not the code for a third visit, a long session, or a hard session. You use it when a patient is in a crisis state and you do an urgent assessment, a mental status exam and a disposition. This guide shows when CPT code 90839 applies, how 90839 vs 90837 and 90834 compare, and what to document. It is written for psychotherapists and for psychiatrists.

For the full code-by-code reference, see our crisis psychotherapy guide.

What CPT code 90839 and +90840 mean

CPT 90839 is “psychotherapy for crisis,” and +90840 is the add-on for each additional 30 minutes. CMS describes the crisis service as an “urgent assessment and history of a crisis state, a mental status exam, and a disposition” (CMS, Psychotherapy for Crisis, page modified 02/25/2026). CMS lists 90839 as “Psychotherapy for crisis; first 60 minutes” and 90840 as “each additional 30 minutes” (use with 90839). This guide follows CMS. Private payers can set their own thresholds, so confirm with each one.

+90840Crisis, each additional 30 minutes (add-on, with 90839 only)Each additional 30 minutes (CMS)

The 90834 and 90837 ranges are CPT time ranges from our individual psychotherapy guide. The rule that matters: a longer session does not become 90839. Pick the code by what the session was, then by time. A routine session that ran long is 90837. A crisis assessment is 90839.

What you cannot bill with CPT code 90839

CMS says not to bill crisis codes with diagnostic evaluation codes (90791, 90792) or other psychiatric and psychotherapy procedure codes (90785–90899, 90832–90838). That means no 90834 or 90837 for the same crisis time. The Medicare-only crisis codes G0017 and G0018 follow their own site-of-service rule, so confirm it before billing them. Time on the date of service can be non-continuous, and the patient must be present for all or some of the service (contractor article A57520, First Coast, rev. 1/1/2025).

Who can bill it

CMS lists physicians (MDs and DOs), clinical psychologists, clinical social workers, clinical nurse specialists, nurse practitioners, physician assistants, certified nurse-midwives, marriage and family therapists, and mental health counselors. CMS lists hospitals, skilled nursing facilities, physician offices, patient homes, and telehealth when requirements are met.

Psychotherapists: bill 90839 for the first 60 minutes of crisis service and +90840 for each additional 30 minutes (CMS).

Psychiatrists: same codes. Do not bill an E/M code or a psychotherapy add-on for the same crisis time. Learn how E/M differs in our E/M and home-visit code guide.

What to document

Every timed code needs the date of service and the start and end time. For a crisis note, also record:

  • The crisis state and why the session was urgent
  • The mental status exam
  • The disposition (what happened next)
  • Total crisis time, so the +90840 count is clear

See documentation requirements for the full list.

Payer rules differ

These are Medicare facts. Private payers and state Medicaid programs set their own limits, time thresholds and covered providers. Rules also vary by state. Ask each payer, and see the CPT & E/M code guide for the rest of the code families.
