Ohio Medicaid provider enrollment
The Ohio Department of Medicaid (ODM) runs Ohio Medicaid. For provider enrollment and revalidation problems, call the ODM Integrated Help Desk at 800-686-1516 or use the ODM provider enrollment support page. Most members are in managed care plans, so identify the plan before you call. Checked October 2026.
Ohio Medicare: who processes your Part B claims
Ohio Medicare Part B claims go to one Medicare Administrative Contractor (MAC). CGS Administrators is the MAC for Jurisdiction 15, which covers Ohio and Kentucky. Medicare Advantage plans are private plans, so call the number on the member card for those. Medicare, Medicaid and private payers follow different rules.

What to have ready before you call a Ohio insurer
A short, prepared call is faster than a long one. Keep these in front of you before you dial any Ohio insurance companies contact number:
- The patient’s name as it appears on the card, date of birth and address.
- Your individual NPI (National Provider Identifier) and your tax ID.
- The date of service, the CPT code and the total billed amount. See the non-E/M CPT guide and the E/M CPT guide for psychiatrists.
- The claim number from the payer portal, if you have one.
- Ohio Medicaid members: identify the managed care plan first, because each plan has its own provider line and prior authorization process.
- Medicare Part B questions: have your NPI, PTAN and the last five digits of your tax ID ready.
Write down the name of the representative and a call reference number. Phone menus, holds and transfers can add up, and a dropped call means starting over. If you would rather not make these calls, MCM South does it for psychotherapists and psychiatrists.
