TRICARE Billing for Behavioral Health Providers

TRICARE billing is the work of getting paid by the military health program for mental health care, through regional contractors that handle networks, authorizations, and claims. Your rules depend on the patient's plan, the region, and your license type. This guide shows who pays, who can bill, when you need authorization, and how to file.

What TRICARE billing involves: regions and contractors

TRICARE billing does not go to one central payer. The TRICARE regions page lists three regions, East, West, and Overseas, and each has its own contractor. Your patient's address decides the region. Knowing the contractor tells you where to check networks, authorizations, and claims.

The regions changed recently. According to a Defense Health Agency article, six states moved from East to West on January 1, 2025: Arkansas, Illinois, Louisiana, Oklahoma, Texas, and Wisconsin. TriWest replaced Humana Military in those states. The article adds that each provider had to decide whether to continue with TRICARE.

If you practice in one of those six states, recheck every network listing and every saved payer address. Old Humana Military contacts for TRICARE billing in those states are out of date.

Who pays a TRICARE claim?

The regional contractor processes the claim for the patient's region. TRICARE reimburses covered services at the TRICARE-allowable charge, not your full fee. The patient's copayment, cost-share, or deductible is separate. The right contractor is the one for the patient's home region, not yours.

Who can bill: TRICARE billing and behavioral health provider eligibility

TRICARE billing depends on provider type, and counselors in particular have two different pathways.

TRICARE behavioral health provider types (checked 10/10/26)
Provider typeTRICARE ruleBilling impact
Psychiatrist or psychologistOutpatient visits with a network provider in the region need no referral or pre-authorization for Prime patients, except psychoanalysis and outpatient substance use disorder (SUD) therapyConfirm network status for the patient's region
TRICARE certified mental health counselor (TCMHC)Independent provider licensed at the highest level in the jurisdiction; no physician referral or oversight neededMust meet exam, degree, and supervised-hours rules
Supervised mental health counselor (SMHC)A physician refers the patient and oversees the careEach claim certifies that results go in writing to the referring physician
Pastoral counselorA doctor must supervise the careSupervision must be documented

The TRICARE mental health appointments page states the Prime referral rules. The TCMHC and SMHC criteria come from a TriWest provider page. A TCMHC needs a master's or higher from a CACREP-accredited counseling program, passage of the NCMHCE or a DHA-approved equivalent, and two years of post-master's supervised practice with at least 3,000 clinical hours and 100 hours of supervision.

An SMHC needs a master's in mental health counseling or an allied field, two years of post-master's experience, and licensure or certification. The same page describes older TCMHC rules for earlier dates, so check which version applies to your credential.

Other license types, such as social workers and marriage and family therapists, are not covered by the pages used here. Check the regional contractor's provider types page for your license before you promise a patient anything about TRICARE billing.

Credentialing paperwork is a separate task from the claim. Our credentialing and network status guide and credentialing service page explain the basics.

Referrals and authorization in TRICARE billing

Authorization rules in TRICARE billing depend on the plan. Look up the plan first, then the service.

Active duty and activated Guard or Reserve

TRICARE says these patients should get mental health care at a military hospital or clinic when possible. Seeing a civilian TRICARE-authorized provider requires a referral and pre-authorization, even in TRICARE Prime Remote.

TRICARE Prime

Outpatient visits with a network psychiatrist or psychologist in the region need no referral or pre-authorization, except for psychoanalysis and outpatient SUD therapy. Non-network visits can trigger point-of-service fees. Inpatient care needs a referral and pre-authorization from the regional contractor.

TRICARE Select and other plans

These plans, including Select, Reserve Select, Retired Reserve, and Young Adult Select, need no referral for most mental health appointments. Patients may see any TRICARE-authorized provider, network or not. Inpatient care needs pre-authorization. TRICARE For Life follows Medicare's mental health rules until those benefits run out.

Services that need pre-authorization

A West Region military clinic referral page says routine in-network outpatient therapy, medication management, and telehealth need no referral. It lists pre-authorization for non-emergency inpatient care, intensive outpatient programs, partial hospitalization, applied behavior analysis, TMS, ECT, Spravato, and residential treatment. That list comes from one region's clinic, so confirm your own contractor's current list before scheduling.

The practical rule for TRICARE billing: never assume. Check the patient's plan, the service, and the region, then record the authorization number on the claim.

6 steps for clean TRICARE billing

  1. Find the region. Use the patient's home address to pick Humana Military, TriWest, or the overseas contractor.
  2. Confirm the plan. Prime, Select, and TRICARE For Life follow different referral rules.
  3. Confirm your status. Check that you are a network or TRICARE-authorized provider for your license type.
  4. Get authorization when required. Save the referral or authorization number with the chart.
  5. Submit the claim. The contractor accepts the CMS-1500, the UB-04 (CMS-1450), or DD Form 2642, as appropriate.
  6. Track the clock. File within one year of the date of service and read the remittance when it arrives.

Behavioral health practices usually file on the CMS-1500 claim form. The TRICARE claims page says your provider usually files for you in the U.S. and that most claims are processed within 30 days. It also says patients must file their own claims if they see a TRICARE nonparticipating provider, so know your status.

Filing deadlines and exceptions

The TRICARE Operations Manual chapter on claims filing (2024-05-30 version) states that claims must reach the contractor within one year of the date of service. The same chapter limits consideration to services received within six years before the request. The tricare.mil claims page adds three years for care in other overseas areas.

The manual lists exceptions: retroactive eligibility or pre-authorization, administrative error by a DHA or contractor employee, mental incompetency, other health insurance, and dual Medicare eligibility. For the last two, the claim must be filed within 90 days of the other payer's decision. Do not count on an exception. Read the claims processing terms and send claims early.

When a claim denies, match the codes to the remittance using the electronic remittance advice guide. Then use the claim denials and appeals resource and your contractor's appeal process.

TRICARE, Medicare, private, and VA: keep the rules apart

TRICARE

TRICARE billing runs through a federal military health program and regional contractors. Everything above belongs here.

Medicare and TRICARE For Life

TRICARE For Life follows Medicare's mental health rules until Medicare benefits run out, then adds its own rules for psychoanalysis and SUD therapy. If a patient has both, check Medicare enrollment and assignment first.

Private insurance

Commercial plans set their own contracts, and TRICARE billing rules do not carry over to them. A patient with TRICARE and private coverage may involve two payers, and one is billed first. Review private insurance guides and the in-network vs out-of-network credentialing page. The TRICARE Operations Manual treats other health insurance as a filing-deadline exception, so record both payers' dates.

VA Community Care

VA Community Care is a different program from TRICARE billing. According to the VA Community Care eligibility page, a veteran needs VA health care team approval before seeing a community provider, with exceptions such as urgent or emergency care. Care must come from an in-network community provider. For mental health, the access standards are a 30-minute average drive or a 20-day wait. How a practice joins the VA network was not covered by the pages used here, so confirm it on va.gov.

What to do Monday morning

  1. List your TRICARE patients by plan and region, and flag any in Arkansas, Illinois, Louisiana, Oklahoma, Texas, or Wisconsin.
  2. Check your network listing with Humana Military or TriWest for each license on your team, since TRICARE billing follows network status.
  3. Write the pre-authorization list from your contractor on the front-desk TRICARE billing checklist.
  4. Set a one-year filing reminder on every TRICARE claim at submission.
  5. Ask for help if TRICARE billing is eating staff time. See the billing services MCM South offers psychotherapists and psychiatrists.

Frequently asked questions

Who processes TRICARE billing claims for therapists?

The regional contractor does. Humana Military handles the East Region, TriWest Healthcare Alliance the West Region, and International SOS the Overseas Region. TRICARE For Life claims go to WPS. Use the patient's home region, not your office location.

Do I need a referral for TRICARE billing on a therapy visit?

It depends on the plan. TRICARE Prime patients need none for outpatient visits with a network psychiatrist or psychologist, except psychoanalysis and outpatient substance use therapy. Active duty patients seeing civilian providers need a referral and pre-authorization.

What is the TRICARE billing deadline?

TRICARE says to file within one year of service in the U.S. and territories, and within three years elsewhere overseas. The operations manual lists limited exceptions. File early, because contractors must deny late claims unless an exception applies.

Can any licensed counselor bill TRICARE?

Not automatically for TRICARE billing. TRICARE separates certified mental health counselors, who can bill independently, from supervised counselors, who need a physician referral and oversight. Each type has degree, exam, and supervised-hours criteria. Confirm your credential with the regional contractor.
