Rochester Mental Health Billing Service: 5 Easy Checks

A Rochester mental health billing service is an outside company that sends insurance claims, posts payments and follows up on denials for therapy and psychiatry practices in Rochester, New York. It helps most when a practice is adding clinicians, offering video visits and carrying a mix of Blue, commercial, Medicare and Medicaid plans. This guide gives five checks to run, plus the New York rules behind them.

Rochester: growing a practice across several payers

The U.S. Census Bureau’s QuickFacts page for Rochester lists a median household income of $47,213 for 2020 to 2024 in 2024 dollars. That figure says nothing about any one client’s coverage. A Rochester mental health billing service should read each client’s card, not the zip code.

Excellus and the commercial mix

Excellus BlueCross BlueShield lists Rochester as one of the regions it serves on its home page. Its footer calls it “an independent licensee of the Blue Cross Blue Shield Association.” Treat it as its own payer, with its own contract and its own provider portal.

Commercial plans sit next to it, and each has its own rules. Keep a payer sheet for every plan you bill. Run insurance verification before each first visit, so the plan on the card is the plan on the claim.

Adding a clinician

A new clinician is not billable with a payer on the first day of work. Each payer sets its own start date after credentialing. Record that date, in writing, for every plan.

Until the date is confirmed, hold or reschedule those visits. A Rochester mental health billing service should keep one grid: clinician by payer, with status and effective date.

Video visits and psychiatry

If you offer video visits, read our telehealth billing guide so the place-of-service code and modifier match each payer’s rule. Psychiatrists bill evaluation and management (E/M) visits, often with a psychotherapy add-on code. Our psychiatry billing services page covers the pairing.

If a clinician is out of network with a plan, ask whether the service bills out-of-network claims. MCM South bills out-of-network claims for clients.

Five checks a Rochester mental health billing service should run

Run these five checks for every new clinician and every new payer, and again when a payer changes its rules.

CheckWhat to confirm
1. Blue and commercial enrollmentEach plan’s credentialing status and effective date for every clinician, in writing, with Excellus treated as its own payer
2. Medicare and Medicaid enrollmentEach clinician’s Medicare record, and Medicaid enrollment where you take Medicaid clients
3. Member matchName, date of birth, member ID and plan on the claim match the card on the date of service
4. Claim setupBilling and treating clinician names, NPIs and tax ID match each payer’s file; video visit codes and psychiatry add-on codes match the payer’s rule
5. Filing clocksMedicare’s 12 months, New York’s 120-day general rule for insurers and each contract’s own window

Ask what happens to visits before an effective date. Get the answer in writing.

New York rules behind a Rochester mental health billing service

Keep private, public and state rules apart. A claim follows the rules of its payer type.

Private payers

Excellus is an independent licensee, so do not apply another state’s Blue plan rules to it, and do not merge it with other New York Blue licensees. Its site shows separate logins for members, employers, brokers and providers. Use the provider login for manuals and claim tools. Your contract controls the rest.

Read each contract with our payer contract compliance guide, and keep payer contacts in our insurance companies contact directory.

Medicare and Medicaid

The Centers for Medicare & Medicaid Services (CMS) lists National Government Services, Inc. as the A/B Medicare Administrative Contractor for Jurisdiction K. The page lists New York among its states and shows the contract as “Fully Implemented.”

Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies.

The New York State Department of Health says adult Medicaid managed care enrollees get behavioral health benefits through their plan, on its behavioral health managed care page. For clients on Medicaid, bill the plan on the card, and get each plan’s filing window in writing. The state’s general Medicaid page links to provider information.

State rules

The New York Department of Financial Services (DFS) health insurance page says insurers must pay claims within 30 days of receipt if submitted online or by email, and within 45 days if submitted on paper or by fax. It says late payments accrue interest at the greater of 12 percent a year or the state corporate tax rate.

The same page says providers generally must submit claims within 120 days after the date of service. It says a provider can ask for reconsideration of a claim denied only for late filing. It also says DFS lacks jurisdiction over self-funded plans and Medicare, so check the plan type before you cite these rules.

For a prompt pay problem, the page says to confirm first that the insurer received the claim, then file through the DFS portal. Confirm the right route with your counsel before you file.

How do I choose a Rochester mental health billing service when my practice is growing?

Choose a Rochester mental health billing service that can show, for each clinician, which payers are active and from what date, and that bills Blue, commercial, Medicare and Medicaid claims by each payer’s rules.

Ask for your clean claim rate, a list of denial reasons and a sample monthly report. Use our guide to CARC and RARC codes to check how it reads remittances. Get fee terms in writing, and confirm in writing that the service bills in New York.

What to do Monday morning

  1. List every clinician with each payer’s enrollment status and effective date.
  2. Add a plan type column to your client list: Excellus, other Blue, commercial, Original Medicare, Medicare Advantage or Medicaid.
  3. Check that each clinician’s Medicare record is current before the next Medicare client visit.
  4. Write each payer’s filing window next to its name, using your contract.
  5. Pull your last remittance and note the top three denial reasons.
  6. Send the five checks to each Rochester mental health billing service you are considering.

The short version: Rochester mental health billing service

A Rochester mental health billing service earns the job by confirming each clinician’s enrollment before claims go out, treating Excellus as its own payer, routing Medicare and Medicaid claims correctly and tracking every payer’s clock. Get the answers, reports and fees in writing.

FAQ

Does a billing service need to be in Rochester?

No. Claims and reports move electronically, so a service can work with practices in Rochester and across New York. New York payer knowledge, written terms and clear reporting matter more than an address. Ask the service to confirm in writing that it bills in New York.

Is Excellus the same as other Blue plans?

No. Excellus’s site describes it as “an independent licensee of the Blue Cross Blue Shield Association.” Use Excellus’s own manuals and your contract, not another Blue plan’s rules.

How fast must a New York insurer pay a claim?

The New York DFS health insurance page says insurers must pay within 30 days of receipt for online or email claims and 45 days for paper or fax claims, with interest on late payments. DFS says it lacks jurisdiction over self-funded plans and Medicare, so check the plan type.

How long do I have to submit claims to a New York insurer?

The DFS page says providers generally must submit claims within 120 days after the date of service. Your contract may set its own window, so read it.

How long do I have to file Medicare claims?

Medicare.gov says claims must be filed no later than 12 months after the service, unless an exception applies. CMS lists National Government Services, Inc. as the contractor for Jurisdiction K, which includes New York.
