Arizona insurance companies contact: private payer provider numbers
Call the provider services number, not the member line. Each row below shows the number, what it is for and the insurer page it came from. If a payer has no behavioral health number listed, use the number on the member ID card.

Arizona Medicaid provider enrollment
The Arizona Health Care Cost Containment System (AHCCCS) runs Arizona Medicaid. For provider enrollment, call 800-794-6862 outside Maricopa County or 602-417-7670 in Maricopa County. Most members are in AHCCCS Complete Care (ACC) plans, so confirm the member’s plan before you call. Source: AHCCCS provider enrollment contact page. Checked October 2026.
Arizona Medicare: who processes your Part B claims
Arizona Medicare Part B claims go to one Medicare Administrative Contractor (MAC). Noridian Healthcare Solutions is the MAC for Jurisdiction F, which includes Arizona. Medicare Advantage plans are private plans, so call the number on the member card for those. Medicare, Medicaid and private payers follow different rules.

What to have ready before you call a Arizona insurer
A short, prepared call is faster than a long one. Keep these in front of you before you dial any Arizona insurance companies contact number:
- The patient’s name as it appears on the card, date of birth and address.
- Your individual NPI (National Provider Identifier) and your tax ID.
- The date of service, the CPT code and the total billed amount. See the non-E/M CPT guide and the E/M CPT guide for psychiatrists.
- The claim number from the payer portal, if you have one.
- Confirm which AHCCCS plan the member is in before calling: Arizona Complete Health, Banner University Family Care, Molina, Mercy Care, BCBSAZ Health Choice or UnitedHealthcare Community Plan.
- For Mercy Care, know whether the member is on the general ACC-RBHA line or the serious mental illness (SMI) line, which has its own 24/7 number.
- Medicare Part B questions: have your NPI, PTAN and the last five digits of your tax ID ready.
Write down the name of the representative and a call reference number. Phone menus, holds and transfers can add up, and a dropped call means starting over. If you would rather not make these calls, MCM South does it for psychotherapists and psychiatrists.
