Connecticut Medicaid Provider Enrollment: Your 2-Part Guide

Connecticut Medicaid provider enrollment is a two-part job for behavioral health. You enroll with the Connecticut Medical Assistance Program (CMAP), and you credential with Carelon Behavioral Health, the administrative services organization (ASO) that runs the HUSKY Health behavioral health network. This guide explains the two parts and what to confirm on the official sites. Status is current as of October 17, 2026. Confirm every item with the state before you act.
How Connecticut Medicaid provider enrollment works
Connecticut Medicaid is called HUSKY Health, and providers enroll through the CMAP secure web portal at ctdssmap.com. State bulletins reference the portal’s enrollment and re-enrollment wizards, including Provider Bulletin 2020-11 and Provider Bulletin 2014-52. The Department of Social Services says each provider is assigned a type and specialty category, and it requires providers to keep all applicable licensing and certification requirements, per its requirements for provider participation in CMAP.
Part 1: CMAP enrollment
Start on the CMAP portal and complete the enrollment wizard for your provider type. Have your NPI, Connecticut license and practice details ready. We could not open the CMAP enrollment pages directly while researching this guide, so confirm the current steps, the enrollment criteria matrix and any document requirements on ctdssmap.com before you start.
Re-enrollment follows the same portal. The state bulletins say providers receive a re-enrollment letter with an application tracking number, that the due date shows on the portal account home page, and that providers who miss the deadline are disenrolled and cannot bill for dates of service after deactivation. Those bulletins are from 2014 and 2020, so check the portal for the current cycle.
Part 2: Carelon behavioral health credentialing
A 2026 Connecticut Behavioral Health Partnership overview says the state agencies contract with Carelon Behavioral Health as the behavioral health ASO, and that Carelon helps providers with credentialing, maintains provider files and verifies active enrollment. A separate January 2026 Carelon overview lists the individual practitioners in the HUSKY behavioral health network: psychiatrists, psychologists, APRNs, LCSWs, LMFTs, LPCs, LADCs and BCBAs.
Neither document states which part must come first. Do both, and ask Carelon whether your CMAP enrollment must be active before it processes your credentialing application. Carelon’s national join-our-network page describes a different process and does not mention Connecticut, so do not apply it to HUSKY.
What to confirm before you apply
- The current CMAP enrollment steps and document list on ctdssmap.com.
- Whether your license type enrolls as an individual practitioner or through a group.
- Carelon’s current credentialing steps for HUSKY providers.
- Your re-enrollment due date on the portal account home page.
Medicaid rules are separate from Medicare and private insurance
Connecticut Medicaid enrollment is a state process. Medicare runs through CMS, covered in our Medicare enrollment guide, and private plans credential you on their own, as our guide to getting credentialed with insurance explains.
| Step | Where it happens | What to confirm |
|---|---|---|
| State Medicaid enrollment | The state Medicaid agency’s provider enrollment portal | Your provider type, enrollment status and effective date |
| Managed care plan credentialing | Each plan’s credentialing department | Network status and contract effective date, which are separate from state enrollment |
| Medicare | CMS and your Medicare Administrative Contractor | Medicare enrollment is separate from Medicaid |
Practical tips for connecticut medicaid provider enrollment
Plan your connecticut medicaid provider enrollment before you need to bill, because the application sits behind the state’s own login. Keep a copy of every connecticut medicaid provider enrollment confirmation, since the state’s records are what a plan checks later. If you hand connecticut medicaid provider enrollment paperwork to a billing partner, keep your own login and records.
