Illinois Medicaid Provider Enrollment: 7 Easy Prep Steps

Illinois Medicaid provider enrollment runs through IMPACT, the Illinois Department of Healthcare and Family Services (HFS) system. A therapist or psychiatrist enrolls there before billing Medicaid or joining a HealthChoice Illinois plan network. This guide walks through the prep steps HFS lists, which behavioral health provider types the notices name, and how revalidation works. Status is current as of October 17, 2026. Confirm every item on the HFS site before you act.
How Illinois Medicaid provider enrollment works
HFS describes IMPACT as the Illinois Medicaid Program Advanced Cloud Technology system. The HFS IMPACT page links to the login and to a preparing-to-enroll page, and the application itself sits behind the login.
HFS took over primary responsibility for provider credentialing through IMPACT effective January 1, 2018, according to HFS notices from May 2019 and October 2022. That is why the state, not each plan, is where you start.
The 7 prep steps HFS lists
The HFS preparing-to-enroll page lists these steps to finish before you enroll:
- Get a 10-digit NPI from CMS.
- Select a taxonomy number that matches your education, license and services.
- Send a certified W-9 with the IRS assignment letter for group, sole proprietor and similar enrollments. HFS says rendering-only providers do not submit a W-9.
- Keep your Illinois licenses and certifications active. IMPACT verifies credentials with bodies such as DFPR and DEA.
- Have an active email address and a compatible browser. HFS names Edge and Firefox.
- Create your IMPACT login.
- Read the Medical Provider Handbook.
HFS also publishes a provider enrollment readiness checklist that points you to the provider types and specialties spreadsheet.
Which behavioral health provider types HFS names
A November 30, 2021 HFS notice says practitioners must enroll with the right provider type, specialty and subspecialty and hold a state license. It lists the provider type Behavior Health Services with two specialties: Licensed Clinical Professional Counselor and Licensed Marriage and Family Therapist.
The same notice does not mention licensed clinical social workers or psychologists, and we could not confirm the individual-practice pathway for LCSWs, psychologists, psychiatrists or APRNs. Check the HFS provider types spreadsheet or call the IMPACT help desk before you pick a category.
HealthChoice Illinois plans and the provider roster
HFS standardized a provider enrollment roster that all HealthChoice Illinois plans use. HFS says new providers complete it when they are added to a plan network listing, and that sending it to the plan at the same time as your IMPACT application may speed up credentialing. The notices do not say whether each plan runs a separate contracting step, so confirm that with each plan you want to join.
Revalidation in Illinois
HFS says CMS requires states to revalidate every enrolled Medicaid provider at least every five years, and each service location revalidates separately. On the HFS revalidation page, the notices arrive 90 and 30 days before the cycle ends, and missing the deadline leads to disenrollment that cannot be backdated.
The HFS revalidation FAQ says the due date shows in IMPACT under Basic Information and that Medicare revalidation does not replace Illinois revalidation.
Medicaid rules are separate from Medicare and private insurance
Illinois Medicaid enrollment is a state process. Medicare enrollment runs through CMS, covered in our guide to Medicare enrollment for therapists, and private plans credential you on their own, as our guide to getting credentialed with insurance explains. For Illinois payer phone lines, see our Illinois insurance companies contact page.
| Step | Where it happens | What to confirm |
|---|---|---|
| State Medicaid enrollment | The state Medicaid agency’s provider enrollment portal | Your provider type, enrollment status and effective date |
| Managed care plan credentialing | Each plan’s credentialing department | Network status and contract effective date, which are separate from state enrollment |
| Medicare | CMS and your Medicare Administrative Contractor | Medicare enrollment is separate from Medicaid |
Practical tips for illinois medicaid provider enrollment
Plan your illinois medicaid provider enrollment before you need to bill, because the application sits behind the state’s own login. Keep a copy of every illinois medicaid provider enrollment confirmation, since the state’s records are what a plan checks later. A practice that tracks its illinois medicaid provider enrollment dates avoids surprise denials. If you hand illinois medicaid provider enrollment paperwork to a billing partner, keep your own login and records.

HFS says group, sole proprietor and similar enrollments send a certified W-9 with the IRS assignment letter, and that rendering-only providers do not submit one. Confirm which category you fall into on the HFS page.
How often do I revalidate in IMPACT?
HFS says at least every five years, with notices 90 and 30 days before the cycle ends. Your due date shows in IMPACT under Basic Information.
