Credentialing Checklist for Therapists: 12 Items to Gather

Desk with lamp and notes where a therapist assembles a credentialing checklist for therapists

A credentialing checklist for therapists saves weeks of back-and-forth. Credentialing is the process insurers use to verify your license, training and coverage before they add you to a network. Most delays come from a missing or expired document, not from the payer. Gather the items below before you submit your first application.

What belongs on a credentialing checklist for therapists?

Use these 12 items as your starting file. Payers may ask for more, so treat this as a floor.

  1. Individual NPI, confirmed in NPPES.
  2. Current state license, with number and expiration date.
  3. Degree and graduate school details.
  4. Current CV with no unexplained gaps.
  5. Malpractice insurance face sheet showing coverage dates.
  6. W-9 with the name and tax ID you will bill under.
  7. Practice address and any service locations.
  8. Business entity details, if you bill as a group or LLC.
  9. Your DataSpring profile ID (formerly CAQH), current and attested.
  10. Supervisor or collaborative agreement details, where your license requires them.
  11. DEA and state controlled-substance certificates, for prescribers.
  12. Board certification and a malpractice claims history summary, where applicable.

Our credentialing terms glossary defines each term. If your DataSpring profile is not set up yet, follow our guide to the DataSpring profile for therapists.

How do you keep the file organized?

Make one folder with a scan of each item and a sheet listing every expiration date. Name files by item and year. Set reminders 90 days before a license or insurance policy expires. A renewed license that never reached your profile is a common reason for a payer to pause an application.

Private payers vs Medicare vs Medicaid

Private payers. Each plan has its own application and contract. Many pull from your profile. Read our credentialing vs enrollment explainer so you know which step you are on.

Medicare. Enrollment goes through PECOS. The CMS enrollment page explains who must enroll.

Medicaid. Rules and portals differ by state, and managed care plans add their own steps. This varies by state.

What should you check before you submit?

  • Names and addresses match across your license, NPPES and applications.
  • Every upload is current and legible.
  • The tax ID matches your W-9.
  • You know whether you are applying as in-network or planning to stay out of network. See in-network vs out-of-network credentialing.

Frequently asked questions

How early should I start credentialing?

Start before you plan to see insured clients. Timelines vary by payer and state, so build in extra time and keep your file ready.

Do I need every item on this checklist?

Not always. Prescribers need DEA and controlled-substance certificates, and some licenses require supervision details. Check each payer’s application.

Is credentialing the same as enrollment?

No. Credentialing verifies your qualifications. Enrollment adds you to a payer’s network and billing system. They often overlap in practice.

Can I bill insurance while credentialing is pending?

Only if the payer allows it. Do not assume retroactive coverage. Confirm effective dates in writing with each payer.

If you are weighing whether to hand this work off, read part-time biller vs billing service. A credentialing checklist for therapists only works if someone owns it. Pick one person, whether that is you, an office manager or a service, and give them the folder and the calendar.

Treat this credentialing checklist for therapists as a living file. Update the credentialing checklist for therapists whenever a license renews, and share the credentialing checklist for therapists with whoever submits your applications.
