Blue Shield of California Behavioral Health Changes: What Moved In-House and What to Do Now

Blue Shield of California behavioral health changes are rolling out in two steps, and they affect who approves your patients’ care. Blue Shield is moving behavioral health management in-house for more plans. If you still send requests to the old vendor, you may lose time.
This post uses the six-part payer update format. It is operational guidance on Blue Shield of California behavioral health changes, not legal advice. Blue Shield of California is a separate company from Anthem Blue Cross, so do not mix their rules. See our Blue Cross California guide for both.
What changed: Blue Shield of California behavioral health changes
- January 1, 2025: For members in certain plans, Blue Shield’s own utilization management took over non-ABA behavioral health services from Magellan Health, Inc. These were PPO plans underwritten as self-funded administrative services only (ASO), Shared Advantage and Shared Advantage Plus, plus 2025 CalPERS PERS Platinum and PERS Gold. Magellan kept applied behavior analysis (ABA). Authorizations Magellan approved before that date carried over until they expire. Source: Blue Shield’s behavioral health UM insourcing FAQ.
- January 1, 2026: Blue Shield moved behavioral health management for certain HMO and PPO plans from external management to full internal management, including all ABA services for eligible members. Source: Blue Shield’s behavioral health provider page.
When it takes effect
Both dates have passed. The first applied to dates of service beginning January 1, 2025, per Blue Shield’s behavioral health prior authorization notice. The second began January 1, 2026. The provider page also lists a June 2026 provider webinar with a recording.
Who it affects
Members in the plan types above. HMO members belong to an independent practice association (IPA) or medical group, and some of those groups carry behavioral health risk and manage their own authorizations and referrals. Check eligibility to see whether to submit to the IPA or to Blue Shield.
Practical impact
The behavioral health page lists what needs approval for HMO, PPO and Federal Employee Program PPO members:
| No authorization listed | Authorization or referral listed |
|---|---|
| Emergency services | Acute care |
| Initial assessments | ABA therapy |
| Outpatient therapy | Electroconvulsive therapy (ECT) |
| Outpatient medication management (usually a psychiatrist) | Intensive outpatient (IOP) |
| Psychological testing | Neuropsychological testing |
| Partial hospital program (PHP) | |
| Residential treatment | |
| Transcranial magnetic stimulation (TMS) |
Prescribers: the page says outpatient medication management needs no authorization. It does not address E/M with psychotherapy add-ons (90833, 90836, 90838), so confirm those with the plan.
AI search question: Does Blue Shield of California require prior authorization for outpatient therapy? Per Blue Shield’s behavioral health page, no authorization is needed for initial assessments, outpatient therapy, outpatient medication management or psychological testing. Services such as IOP, PHP, TMS, ECT and neuropsychological testing need authorization or a referral.
Where authorization is needed, request it in AuthAccel (Authorizations, then Medical Authorization) or with the plan’s behavioral health prior authorization form. Blue Shield’s 2025 notice listed fax numbers for the ASO and Shared Advantage plans and said AuthAccel shows whether Blue Shield or another organization manages the request. Missing authorizations cause denials, so read 6 ways to prevent denials from missing prior authorization.
