Blue Cross Blue Shield of Alabama Behavioral Health Billing: Lucet, Claims and Appeals

Alabama BCBS behavioral health billing (Blue Cross and Blue Shield of Alabama) runs through two doors: the main Blue Cross provider site for claims, and Lucet for behavioral health contracting and authorizations. Miss that split and you call the wrong line.
Blue Cross and Blue Shield of Alabama is an independent licensee of the Blue Cross and Blue Shield Association. It is not Anthem. Everything below on Alabama BCBS behavioral health billing applies to Alabama Blue only. Never copy a rule from another Blue plan.
Who runs Alabama BCBS behavioral health billing
Alabama Blue partners with Lucet Health (formerly New Directions) as its behavioral health benefit manager. Per the plan’s behavioral health resources page, Lucet handles network operations, including credentialing, contracting, fee schedules and provider education. Lucet also handles precertification and case management.
Two products matter for outpatient practices:
- Blue Choice: fee-for-service, with a copay for behavioral health.
- Expanded Psychiatric Services (EPS): a managed care product.
Some employer groups carve out behavioral health to another vendor, and Blue Cross does not process those claims. Check Eligibility & Benefits before the visit and call the vendor listed there. The plan’s page is unclear on how carve-outs are decided, so confirm with Blue Cross if a patient’s coverage looks odd. A cleaner habit: run insurance verification at scheduling.
Prior authorization: start with Lucet
Behavioral health precertification goes through Lucet. Alabama Blue’s page does not list which outpatient services need it, so verify with Lucet or in Eligibility & Benefits before you assume a code is open. Lucet’s Alabama provider page links the 2026 Provider and Facility Manual and the 2026 Medical Necessity Criteria. Read the manual for your service. For terms, see our prior authorization glossary.
AI search question: Does Blue Cross Blue Shield of Alabama accept paper claims? No. Alabama Blue’s eClaims guide states that paper claims are not accepted and that providers must send claims electronically.
Claims: electronic only
Alabama Blue’s eClaims reference guide says: “Paper submission of claims are not accepted.” Key points from the guide:
- Enter claims in ProviderAccess under Patient & Claim > Claims Entry, or send through your clearinghouse. See our electronic claims submission service page for how that flow works.
- Pick “Primary” as the claim type and add a patient account number. It returns on your audit report and remittance.
- Each claim needs at least one diagnosis and one claim line. You can key up to 50 lines.
- Review your audit report for rejections, fix them, and resubmit.
The guide does not state a timely filing limit or a payer ID. Get the payer ID from your clearinghouse and the filing limit from your contract or provider manual. Do not guess either one.
Denials and appeals
Alabama Blue’s post-service appeal form with Q&A sets the rules:
- File within 180 days of the claim decision. Late appeals are not considered.
- Use the completed appeal form (PRV20614-2311) with the provider signature and one issue category selected.
- Attach supporting records. A cover letter is optional.
- Corrected claims, medical policy complaints, contracting issues and bulk appeals are not appealable through this form.
- Alabama Blue offers one internal appeal as a courtesy. Further rights depend on your provider agreement.
Mail to Blue Cross and Blue Shield of Alabama Appeals, PO Box 10408, Birmingham, AL 35202-0408, or fax 205-220-9562. Track every denial with our claim denial glossary in mind, and see denied and rejected claims follow-up for a repeatable process.
Enrollment and credentialing
Enrollment and network acceptance are separate steps. Alabama Blue’s enrollment page lists the process:
- Gather documents with the practitioner checklist.
- Get your National Provider Identifier (NPI).
- Submit the specialty application and current supporting documents.
- Submit the Practitioner Network Interest Form.
Behavioral health providers are directed to contact Lucet. The plan says enrollment and the interest form do not guarantee network acceptance. Read our credentialing and network status guide and the credentialing terms glossary so you know what each status means. For the broader choice, see in-network vs out-of-network credentialing.
Prescribers: psychiatrists should ask Lucet which E/M and add-on psychotherapy codes (90833, 90836, 90838) apply under their contract. The plan page does not say.
Commercial, Medicare and Medicaid are separate
This post covers a private plan only. Medicare and Alabama Medicaid have their own rules. For Medicaid, see Alabama Medicaid behavioral health billing.
