Illinois CPT 96127 Modifier: U1 and U2 Explained

Quiet counseling room where Illinois providers apply the Illinois CPT 96127 modifier

The Illinois CPT 96127 modifier rule is now in effect: Illinois requires a modifier on CPT 96127 claims. The Illinois Department of Healthcare and Family Services (HFS) says claims for dates of service on or after August 1, 2026 need modifier U1 or U2, or they are rejected. Here is what changed and what to do about it. This is billing guidance for Illinois Medicaid, not legal advice.

What changed with the Illinois CPT 96127 modifier?

HFS published a provider notice dated July 7, 2026 titled Coding Update for Behavioral Health Screenings. The notice covers CPT 96127, brief emotional or behavioral assessment. Our CPT 96127 billing guide explains the code itself.

What do U1 and U2 mean for the Illinois CPT 96127 modifier?

Modifier Meaning in the HFS notice
U1 Negative screening result
U2 Positive screening result with a documented follow-up plan

Choose the modifier from the result in your note. If the screen is positive, your note should show the follow-up plan. See documentation requirements and CPT modifiers.

Who does it apply to?

The notice applies to Illinois Medicaid fee-for-service claims and to HealthChoice Illinois managed care organization (MCO) claims. Eligible provider types listed include LCSWs, licensed clinical psychologists, behavioral health clinics and community mental health centers. Read the full list in the notice. Check with each MCO for its own claim questions. See Illinois Medicaid billing and Medicaid managed care.

What happens if you leave the modifier off?

The notice says claims for CPT 96127 without U1 or U2 are rejected. For fee-for-service claims, the error code is L54, Missing or Invalid Modifier. A rejection is not a denial, so you correct and resubmit. See claims denied or rejected by insurance and denied and rejected claim follow-up.

What did not change?

The notice says depression screening codes G8431 and G8510 are unchanged. It also ties behavioral health screening to EPSDT (Early and Periodic Screening, Diagnostic and Treatment) well-child visits using age-appropriate tools. That is a Medicaid rule for Illinois, not for private payers or Medicare.

Private payers vs Medicare vs Medicaid

Illinois Medicaid: modifier U1 or U2 is required, per HFS. Private payers: each plan sets its own modifier rules for 96127, so check each payer. Medicare: follow CMS guidance. Do not assume the Illinois rule applies elsewhere. Rules vary by state. See state billing guides and Illinois insurance companies contact.

How to document a 96127 screen

Your note should name the tool, record the result and show the follow-up plan for a positive screen. The modifier on the claim must match that result. If you pick U2 without a plan in the note, the claim may not be supported. A short template line in your EHR can prompt the right documentation every time. Our CPT 96127 guide lists other documentation points.

Reconfirm before you rely on this

The Illinois CPT 96127 modifier rule comes from HFS. HFS notices can be updated. Before you change your billing, open the HFS notice and your MCO’s provider manual. Status is current as of October 15, 2026. MCM South works with providers in Illinois and the other states we serve, but this post is general guidance, not legal advice.
