Florida Medicaid Managed Care Billing: 5 Easy Essentials

Florida Medicaid managed care billing is how a practice gets paid for Medicaid members who are enrolled in a health plan instead of fee-for-service Medicaid. The state’s Agency for Health Care Administration (AHCA) says most Florida Medicaid recipients are in its managed care program, so your claims usually go to a plan, not to the state. This guide covers the five things to set up first.

How Florida Medicaid managed care billing is organized

AHCA runs the Statewide Medicaid Managed Care (SMMC) program. It has three parts: Managed Medical Assistance (MMA), Long-Term Care, and Dental. Behavioral health sits in MMA, where AHCA lists community behavioral health as a minimum covered service for every plan. The current contract round, SMMC 3.0, started February 1, 2025 with new plan contracts and a new regional structure. A Florida Senate bill analysis describes nine regions, lettered A through I.

Essential 1: an active Florida Medicaid ID

Providers enroll through the Florida MMIS Provider Portal. AHCA suggests starting with its Enrollment Readiness Tool, which shows your provider type, enrollment category and screening level. Categories include fully enrolled, limited, and rendering-only (a clinician who works under a group’s billing privileges). Every provider completes Level 2 background screening and revalidates periodically. Florida Medicaid managed care billing starts with this ID.

Plans check this. A Sunshine Health notice dated October 31, 2022 told network providers that a Medicaid ID shown as “Registered” on AHCA’s Provider Master List makes claims deny. Look up your own status now. Molina’s Florida billing guidelines also say the taxonomy on your claim must match your AHCA enrollment record.

Essential 2: a contract with every plan you bill

AHCA’s health plan resources page links provider guides for Aetna, Children’s Medical Services, Clear Health Alliance, Community Care Plan, Florida Community Care, Humana, Molina, Simply Healthcare, Sunshine and United Healthcare. Plans are organized by region, so list the plans your patients actually have, then apply to each network. Each plan is a separate contract, so Florida Medicaid managed care billing is never one setup. Joining a plan’s network is separate from AHCA enrollment. Our credentialing service page explains the process.

Florida Medicaid managed care billing rules: check the member, read the plan, track it

Each plan sets its own filing window. Molina’s Florida guidelines (a June 2025 deck) say claims go in within six months of the outpatient date of service, and within 90 days of the primary payer’s final decision when Molina is secondary. Treat that as one plan’s rule, not Florida’s rule.

PlanFiling windowCorrected claimsWhere you confirmed it
Molina (Florida Medicaid)Six months from the outpatient date of serviceSix months from date of service or latest adjudicationBilling guidelines deck, 6/30/25

Check each row against the claim remittance advice and CARC and RARC codes when a denial arrives. A filing-limit denial and a missing-ID denial look different, and the fix is different too.

Medicaid, Medicare and private payer rules differ

Do not copy one payer’s settings into another. Florida Medicaid managed care billing follows state policy plus each plan’s manual, while Medicare and commercial plans run on their own rules.

Medicaid

State rule 59G-4.052 adopts the Florida Medicaid Behavioral Health Therapy Services Coverage Policy, dated November 2019 in the rule text. That policy is the starting point. Each plan then adds its own prior authorization list, portal and billing manual.

Medicare

Under Original Medicare, Medicare.gov says claims must be filed within 12 months of the service date. That is a different clock from your Medicaid plans.

Private payers

Commercial plans follow your contract and Florida insurance law. Start with our guide to mental health insurance billing.

How do I bill a Florida Medicaid managed care plan as a therapist?

Keep your AHCA Medicaid ID active, join the plan’s network, confirm the member’s plan for the date of service, and send the claim the way that plan requires, usually through its portal or your clearinghouse. Match the taxonomy to your enrollment record and track the plan’s filing window. Florida Medicaid managed care billing fails most often at the ID and enrollment step, not at the claim form.

What to do Monday morning

  1. Look up your Medicaid ID status on the AHCA Provider Master List and fix any “Registered” status.
  2. List the Medicaid plans your patients carry and mark which ones you are contracted with.
  3. Download each contracted plan’s provider manual and fill in the filing-window grid above.
  4. Pull your denials from the last 90 days and sort them by plan and reason.
  5. Check your clean claim rate by plan so you can see where Florida Medicaid managed care billing breaks first.

The short version: Florida Medicaid managed care billing

Florida Medicaid managed care billing runs through AHCA enrollment and then through each plan. Get the ID right, contract with the plans you need, and keep a grid of their filing and corrected-claim rules.

FAQ

Do Florida Medicaid managed care plans pay behavioral health claims directly?

Yes. Behavioral health is a covered service inside the Managed Medical Assistance part of the program, so you bill the member’s plan, not the state, when the member is enrolled in one. Confirm the plan on the date of service.

Do I need a Medicaid ID to join a plan’s network?

Plans expect an active Florida Medicaid ID. A Sunshine Health notice from October 2022 said providers listed as “Registered” would see claims deny. Check your own status with AHCA and with each plan.

Is there one timely filing limit for all Florida Medicaid plans?

Do not assume one. Molina’s 2025 billing guidelines say six months for outpatient claims, but each plan publishes its own manual. Record each plan’s window and the date you checked it.

Are Medicaid managed care rules the same as Medicare rules?

No. Original Medicare has its own 12-month filing limit and rules, and Medicaid plans follow state policy plus their own manuals. Keep the rules in separate labeled notes.
